- Physician Statement of Work Project: On-Site Physician "THIS IS A SUBCONTRACTING OPPORTUNITY" The Little Rock Job Corps Center operated by Adams and Associates located at 6900 Scott Hamilton Dr. Little Rock, Arkansas has 300 + students enrolled in our residential and non-residential training facility. Little Rock Job Corps is currently soliciting requests for quote for the following on-site medical services: Medical Services Hours per week Number of weeks Center Physician 12 48 The interested parties must include the following information with their proposal : •A. Resume •B. References •C. Current medical & DEA licenses, if applicable, for the State of Arkansas All proposals are due on May 7, 2012 by 5:00 p.m. central time. The proposal must be submitted to the Purchasing Agent, Ms. Lavorce Crane by one of three methods: 1.) Documents may be hand delivered to the Center; 2.) documents may be submitted via fax to (501) 618-2595; or 3.) documents may be emailed to crane.lavorce@jobcorps.org.
