- Sole Source D&F document Please complete this RFQ document and return as part of your offer. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FA5205-12-Q-JS03; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-56, DPN 20120330, and AFAC 2012-0330. This acquisition will be other than full and open competition; the North American Industry Classification System (NAICS) code is 511210. The business size standard is $25 Million. The Federal Supply Class (FSC) is 7030. The Standard Industrial Classification (SIC) is 7372. This is a sole source requirement IAW FAR 6.302-1. Please see attached D & F for clarification. CLIN 0001: Annual subscription 1 device, Level 1 support (Perpetual License) Groundwork Monitor Enterprise Perpetual Use with one-year Level 1 Support (SKU: FLXSL1) Quanity - 1100 EA. PLEASE INCLUDE ANY SHIPPING CHARGES FOR ALL CLIN'S TO THE FOLLOWING ADDRESS: 373 SPTS/CC Unit 5001, BLDG 1523 APO AP 96319-5001 Attn: Mr. John Riordon Award shall be made in the aggregate, all or none. EVALUATION CRITERIA: Evaluation of offers will be completed in accordance with FAR 13.106-2. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation and represents the best value to the Government, price and other factors considered. The following factors shall be used to evaluate offers: A) TECHNICAL ACCEPTABILITY - All offers must meet the requirements as set forth in this combined synopsis/solicitation.
