Description
Project 626A4-12-101; Lead and Asbestos Abatement Phase 5 I. General Intention A. Contractor shall furnish all labor, material, equipment, and supervision necessary to complete Project No. 626-A4-12-101, "Lead Based Paint and Asbestos Abatement Phase 5" at the VA Tennessee Valley Healthcare System, Murfreesboro Campus (3400 Lebanon Pike, Murfreesboro, TN 37129) and Nashville Campus, (1310 24th Avenue South, Nashville, TN 37212). Work includes, but not limited to-lead based paint and asbestos abatement, re-insulation, or encapsulation as appropriate and specified by the statement of work and other specification that will be provided in the solicitation. B. Site visit dates and times will be published with the solicitation. C. Prior to commencing work, general contractor shall provide proof that a OSHA certified "competent person" (CP) (29 CFR 1926.20(b)(2) will maintain a presence at the work site whenever the general or subcontractors are present. II. Magnitude of construction The magnitude of this requirement is between $1,000,000 and $2,000,000. III. Place of Performance The place of performance will be at both campuses as follows: VA Tennessee Valley Healthcare System, Murfreesboro Campus (3400 Lebanon Pike, Murfreesboro, TN 37129) and Nashville Campus, (1310 24th Avenue South, Nashville, TN 37212). IV. Set-Aside This is a service disabled veteran owned small business (SDVOSB) set-aside. The SDVOSB must be registered with VETBIZ, CCR and ORCA. Project 626A4-12-101; Lead and Asbestos Abatement Phase 5 V. Statement of Bid Items Statement of Bid Items: A.Remove Lead Based Paint and asbestos containing materials from building(s), pipe chases, floors, interior Walls and ceiling rooms, and other specifically identified areas as indicated in attached drawings, notes, and/or the Statement of Work (SOW). Contractor shall remove all Lead Based paint (LBP) and then restore conditions/re-insulate with paint or other approved surfacing materials as specified in the Statement of Work. Contractor is responsible for field surveying and estimating quantities during bid and/or negotiations. Contractor field survey & estimates shall include (but is not limited to) looking interior walls, floors, ceiling and exterior soffits and eaves. This contract will also include asbestos containing 9x9 tile and mastic in the Nashville VAMC. Nothing in this section may be interpreted as limiting the scope of work otherwise required by this contract and related documents. TASKS PART1:LEAD ABATEMENT A.Work summarized briefly as follows: Summary Listing of Locations and Estimated Amount of Lead Based Paint to Abate Murfreesboro Campus - Alving C. York Location # Bldg Location Hazard Type Linear or SF 1. Alvin C. York VAMC Building 16-Boiler Plant LB Paint Surface - 23,000 SF Steel Girders /Pipes 6,500 SF Ceiling - 4400 SF 2. Alvin C. York VAMC Bldg 29LB Paint Abate or removed all LBP in Bldg 29 B.Pre abatement: Activities including pre abatement meeting, inspection, notifications, permits, submittal approvals, preparations, emergency arrangements, standard operating procedures and other steps necessary to assure minimal aerosolIzation of lead to any employee work location or to the environment during the accomplishment of this abatement. Consideration and design-build methodology as to how to provide cooling to abatement workers and HEPA filtration of any exhausted air needs to be included. C.Abatement Activities including removal and disposal of contaminated waste, record keeping, security, inspection and air monitoring both inside and outside containment where workers or other personnel can be present. Daily site inspection logs and inside outside lead monitor when abatement activities are on-going. Project 626A4-12-101; Lead and Asbestos Abatement Phase 5 D. All LBP abated surfaces be resurfaced with paint as specified by the VA specifications. E.Cleaning and Decontaminating Activities including final inspection and air testing and certification of decontamination. TEM final clearance samples for each abated area will be required in addition to the normal PCM samples. F.Daily Lead Air Monitoring will be conducted using an offsite third party AIHA PAT, NVLAP, or NIST certified laboratory. G. Provide TVHS Engineering Service with two Copies of all permits, laboratory Sampling data, disposal records daily inspection logs and contractor employee training certificates. PART 2: ASBESTOS ABATEMENT 1.Abate all 9x9 inch floor and related mastic 3 South and various smaller locations at Nashville VAMC A.Estimated 6100 SF 2.Abate all texture white ceiling in Chapel at Nashville VAMC A.Estimated 1100 SF 3.Abate all Millboard wrap paper pipe insulation and re-insulation of all millboard wrap paper pipe at Nashville VAMC. A.Estimated 60 linear feet 4.Abate all radiators/ containing transite panels at either VAMC in TVHS. A.Estimated 20 Radiators. 5.Abate all small abatement Assorted ACM Locations 10 square feet or linear feet at either VAMC in TVHS. A.Estimated 20 locations 6.Abate all small abatement Assorted ACM Locations < 25 square feet or linear feet at either VAMC in TVHS. A.Estimated 15 locations Project 626A4-12-101; Lead and Asbestos Abatement Phase 5 7.Abate all small abatement Assorted ACM Locations < 50 square feet or linear feet at either VAMC in TVHS.
