- Project: Workplace Violence Prevention Program in New Jersey Healthcare Facilities NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention, National Institute for Occupational Safety and Health (NIOSH), Division of Safety Research, Analysis and Field Evaluation Branch, 1095 Willowdale Road, Morgantown, WV 26505, hereby announces its intent to issue a purchase order with two option years to the University of North Carolina at Chapel Hill for the purpose of the contractor's principal investigator to provide support to NIOSH in the development and management of a collaborative project entitled "Workplace Violence Prevention Programs in NJ Healthcare Facilities". The purpose of this project is to evaluate the impact of the New Jersey Violence Prevention in Healthcare Facilities Act regarding the reduction of workplace violence related injuries in a representative sample of healthcare facilities throughout the state of New Jersey. Scope of Work: The contractor's principle investigator will collaborate with NIOSH to: participate in the development of study protocols and data collection instruments; participate in the development and execution of study methods; provide guidance on database development, data entry and data cleaning; conduct quality assurance checks of interviews with healthcare facility managers and data abstraction from facility records; participate in project meetings; and participate in the dissemination of project findings, including data analysis. Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the Contracting Officer within 15 days from the posting date of this notice, their written capabilities statement and pricing information in the format they choose, not to exceed 10 pages. Please forward the capability statement and pricing information to Rebecca Mullenax, Purchasing Agent, Reference 000HCCCC-2011-99785, Centers for Disease Control, NIOSH, ATTN: Rebecca Mullenax, MS: GD, 1095 Willowdale Road, Morgantown, WV 26505. All vendors must be registered in the Central Contractor Registry (CCR) prior to an award of a federal contract. The website is: www.ccr.gov. The Government will review any/all capabilities statements and pricing information submitted and determine if other qualified sources do exist that could provide this requirement. Information received in response to this announcement will be used solely for the purpose of determining whether to conduct a competitive procurement. If no affirmative responses are received within 15 days, in accordance with FAR 13.106-1(b)(1), negotiations will be conducted with the University of North Carolina at Chaper Hill, Office of Contracts and Grants, 204 Airport Drive, Suite 2200, Chapter Hill, NC 27599-5023, as the only source and a purchase order will be issued without any additional notices being posted.
