- This is a SOURCES SOUGHT NOTICE. This is NOT a solicitation for proposals. The purpose of this Notice is to obtain information regarding the availability of qualified Small Business medical healthcare service providers, NAICS 621111 for Obstetrics/Gynecological Physician Services to be performed for the Fort Drum New York Medical Activity (MEDDAC) at the OB/GYN Clinic located at the Samaritan Medical Center, Watertown, NY 13601. Service consists of 1 FTE Obstetrics/Gynecological Physician. The anticipated period of performance is 1 August 2012 through 31 July 2013 (base year) and includes four (4) option years 1 August 2013 through 31 July 2017. Place of performance will be the Samaritan Medical Center, Watertown, NY 13601. Interested parties should email Rebecca Ruff, at rebecca.ruff1@us.army.mil not later than 19 April 2012, 4:00 PM EST. All responses must be via email. THIS IS STRICTLY MARKET RESEARCH THAT WILL ASSIST THE DEPARTMENT OF ARMY, NORTH REGIONAL CONTRACTING OFFICE, FORT DRUM CELL, PLAN THEIR ACQUISITION STRATEGY. If your firm can meet the criteria above, please provide the following information. Include responses to all items. Please be specific with examples. 1) Firm Name and Address 2) DUNS Number. 3) A short capabilities narrative on your firm. 4) A detailed description (including size, scope and dollar value) of the firm's experience in performing similar contracts.
