- terms and conditions solicitation 12-249-sol-00015 This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR 12.6 and as a supplemented with additional information included in this notice. The solicitation number is 12-249-SOL-00015 and is issued as a request for quote (RFQ). The solicitation document incorporates provisions and clauses in effect through the Federal Acquisition Circular 2005-53. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510 with a small business size standard of 500 employees. This requirement is a Total Small-Business set-aside and only qualified offerors may submit quotes. If FOB Origin, price delivery for this location: Sells Indian Hospital, 1 Mesquite Road, Highway 86 & Topawa Road, Sells, Az 85634. The DHHS/INDIAN HEALTH SERVICE, Sells Indian Hospital required the following items, BRAND NAME OR EQUAL to the following: 1)GE Panda iRES Infant Warmer or Equivalent, Qty 1 2)GE Care Plus 3000 Infant Incubator or Equivalent, Qty 1 3)Installation Services for the Items noted above, Qty 2 4)Individual Operator on-site in-service training for each of the above items noted, Qty 6 5)Individual Biomedical/Technical Training for each of the above items noted, Qty 2 Interested offerors shall submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions not received within a reasonable time prior to close of the solicitation will not be considered. Failure to comply with the following terms and conditions may result in offer begin determined as non-responsive. New Equipment ONLY. No re-manufactured items allowed. All items must be covered by the manufacturer's warranty. No partial shipments are permitted unless specifically authorized by the contracting officer at the time of award. The selected Offeror must comply with the following item terms and conditions, which are incorporated in the attached solicitation terms and conditions. Offeror must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website: http//www.ccr.gov.
