- Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This notice constitutes as Solicitation AG-04T1-S-12-0008 and is being issued as a Request for Quotation. This notice and any incorporated provisions include those in effect through Federal Acquisition Circular 2005-56. Quotes are being solicited from Small Business Concerns only. Description: Provide and install a service body for Forest Service Cab/Chassis 2406 and provide and install a service body for Forest Service cab/chassis 2407 for the Umpqua National Forest. Note: prospective awardee shall be registered in the Central Contractor Registration (CCR) data base prior to award. Information on registration may be obtained via the internet at: http://www.ccr.gov or by calling 1-888-227-2423. All contractors and vendors must complete the electronic version of the Representations and Certifications in the ORCA system. The ORCA site is http://www.bpn.gov - click on the Online Reps and Certs Applications. Provisions and Clauses: See attached solicitation. QUOTE DUE DATE: Quotes shall be received by May 11, 2012, at 1600 (4:00 pm PST). Quotes may be mailed to Umpqua National Forest, Contracting Office, 2900 NW Stewart Parkway, Roseburg, OR 97470 or may be faxed to 541-957-3223.
