- Notice of Proposed Contract Actions IAW FAR 12 and 13, RFP VA259-12-R-0067 The VISN 19 Grand Junction VA Medical Center (GRJVAMC), Grand Junction, CO, and the NCO 19 Rocky Mountain Acquisition Center contemplate an award of a Firm Fixed Price, Indefinite Quantity Contract for Infectious Disease Provider Services. The proposed contract will be issued for commercial firm fixed price contract types. The period of performance for these contracts will be one year, with four additional, one-year option periods from date of award, which is estimated for June 01, 2012. While price will be a significant factor in the evaluation of offers, the final contract award will be based on best value to include past performance. Request for Proposal VA-259-12-R-0067 will be available for download on or about April 16, 2012. No telephone requests for this solicitation will be allowed. Offers are to be received at the VA SAO West NCO 19 Rocky Mountain Acquisition Center, 4100 E. Mississippi Ave., Suite 900, Glendale, CO 80246 with a prescribed closing date and time within the RFP. The NAICS Code is 62111. Additional Requirements: Contractors shall be required to meet the standards of the Joint Commission and to provide current Infectious Disease licensure and proof of insurance.
