Description
The Department of Veterans Affairs, Stratton VA Medical Center, 113 Holland Avenue, Albany, NY (VISN 2) intends to negotiate a sole source commercial item services contract to Innovative Product Achievements (IPA) 1105 Satellite Boulevard, Suite 300, Suwane, Georgia 30024.. This announcement constitutes the only solicitation; a written solicitation will not be issued. The requirement is for the lease of IPA Scrub-Ex scrub dispensing and receiving machines and associated linen management software and maintenance. Maintenance includes assuring the proper operations of the Scrub Ex machines, including, but not limited to: all transportation, installation, labor, unlimited software updates and support, parts, onsite service, unlimited telephone support, and providing the necessary training to personnel on proper equipment operation. "(2) ScrubEx LV (SE-003) large volume dispenser - accommodate approximately 128 sets. Footprint of not larger than: 16.5"D x 52"W x78"H "(2) ScrubEx LV (SE-0093) large volume receivers - accommodate approximately 150 sets and have the capability of packing. Records a video/picture of each deposit. Footprint of not larger than: 16.5"D x 52"W x78"H "(2) ScrubEx MV (SE-3003) medium volume dispenser & receiver - accommodates dispensing 64 sets and receiving 60 sets. Records a video/picture of each deposit. Footprint of not larger than: 16.5"D x 52"W x78"H Delivery and installation of the equipment is required NLT 45 days from date of award. The period of performance for this requirement is June 1, 2012 through September 30, 2012 with the potential for option years. The proposed contract action is for supplies and service for which the government intends to solicit and negotiate with only one source under the authority of FAR 6.302-1. Only one responsible source and no other equipment or training will satisfy agency requirements; therefore, full and open competition is determined inappropriate for this acquisition. This is a notice of intent and is not a request for competitive quotes. Interested persons may identify their interest and capability by responding to the requirement or submit proposals on or before due date identified below. A determination by the Government not to compete this proposed procurement based on the responses to this notice is solely within the discretion of the Government. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification - Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its proposal. A copy of the provision may be attained from https://www.acquisition.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The offeror shall also provide its Commercial and Government entity code, DUNS number, and Tax Identification number. All Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of the information added to CCR. To register, go to http://www.ccr.gov/. The Federal Contractor Program requires that any contractor receiving a contract in the amount of $25,000 or more, or any subcontractor receiving a contract in the amount of $25,000 or more from such a covered contractor must file a VETS-100 Report on an annual basis.
