Description
Bay Pines VA Healthcare System STAT Blood Courier Transportation Service 1. INTRODUCTION: This sources sought notice is for INFORMATION AND PLANNING PURPOSES ONLY and SHALL NOT be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). The VA is currently conducting market research to locate qualified, experienced and interested potential sources for 24 hours a day, 7 days per week, and 365 days per year Blood STAT Courier Service. The service must include emergency local pickup and delivery of items such as, but not limited to, reagents, blood products, and STAT (emergent) testing that is not performed at the medical center. Only responses from eligible Service-Disabled Veteran-Owned Small Business (SDVOSB), Veteran-Owned Small Business (VOSB), Economically Disadvantaged Women-Owned Small Business (EDWOSB), Women-Owned Small Business (WOSB) and other small business firms will be considered. The applicable North American Industry Classification (NAICS) code for this procurement is 492110 Couriers and Express Delivery Services. Responses will not be considered as proposals, nor will any award be incurred by interested parties in responding to this sources sought announcement. 2. CONTRACTOR EXPERIENCE/PERFORMANCE REQUIREMENTS: The contractor shall pickup and deliver to various locations within the Tampa Bay Area to include, but not limited to, Tampa, St. Petersburg, and Clearwater, Florida. All items must be transported in accordance with all local, state and federal regulations; patient specimen transport contained in a locked transport carrier to protect patient sensitive information and/or sealed container that will be properly packaged by the medical center personnel. Vehicles shall be adequately stocked with at least one "SPILL KIT" for bio-hazard cleanup of leakage or accidental discharge from controlled containers. STAT (emergent) transport request items must be picked up and delivered to the drop off location within 60 minutes from time of initial call. All other STAT laboratory requests items should be picked up within 30 minutes of initial call and delivered to the drop off location within 60 minutes from the time of initial call. The responder must have a 24/7 direct contact dispatch line. Contractor experience must be specific to transporting medical STAT items. Transport of Bio-hazardous materials training is required for all for all employees providing courier services. The Contractor shall develop and maintain a quality control program that ensures courier services are performed in accordance with medical center specifications, including procedures to track, identify, prevent, remedy and ensure non-recurrence of defective services. 3. PERFORMANCE OBJECTIVES: a. Timeliness - 2 or less customer complaints per month for all locations serviced b. Condition of transported items - 2 or less customer complaints per month for all locations serviced 4. SUBMITTAL INFORMATION: Contractors having the skills and capabilities necessary to perform the stated requirements should submit a response of NO more than 15 pages in length, single spaced, 12 point font minimum. The Government will not review any other data, attachments that are in excess to the 15 pages. Companies should provide clear and unambiguous evidence to substantiate their capability to fulfill these requirements. Interested parties shall furnish the following minimum information: 1. Company name and address 2. Company point of contact name, telephone number and email address 3. Company business size and status (i.e., Small Business, Service-Disabled Veteran Owned Small Business, Women-Owned Small Business, etc.), the number of years in business, affiliate information: parent company, joint venture partners, potential teaming partners 4. If the company holds a Federal Supply Schedule (FSS) Contract, list the GSA Contract Number and relevant SINS 5. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills your company possesses to perform services described in the scope of work. 6. Past Experience: Brief summary of the company history relative to similar requirements. Response may provide the following information on a maximum of three similar contracts for which the responder was a prime or subcontractor. a. The name, address, and value of each contract b. The Prime Contract Type, Firm Fixed-Price, or Time and Material c. The name, telephone and address of the owner of each contract d. A description of each contract service and why it is relevant to this requirement. Include difficulties and successes e. Your company's role and services provided for each contract. It is requested that the above information be provided no later than April 27, 2012 at 3:00PM EST. Responses should be emailed to James L. Jones at James.Jones30@va.gov.
