Description

This is a combined synopsis/solicitation for commercial items (services) prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and further written solicitation will not be issued. The solicitation number is VA258-12-R-0180 and is being issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-56, effective 02 April 2012. The applicable NAICS code for this service is 621330, Offices of Mental Health Practitioners (except Physicians), with a small business size standard of $7.0 million. The Vet Center located at 230 S Water Street in Las Cruces, NM 88001, has requirements in the Roswell and Carlsbad geographic areas to provide Readjustment Counseling services (from providers i.e., Psychiatrist, PhD Psychologist, Licensed Social Work, and Unlicensed Counselor [including Registered Nurse]) in accordance with established professional standards, principles, practices and ethics of the Joint Commission and the New Mexico VA Health Care System for providing care to veteran patients and performing duties for the Department of Veterans Affairs. Services shall be in accordance with the Work Statement embedded in the solicitation following. The anticipated Period of Performance for these services is June 15, 2012 through June 14, 2013 with two (2) one-year option renewals. The intent of this combined synopsis/solicitation is to result in two separate Requirements-type contracts with Fixed Prices determined by the category of provider rendering service to the veteran patient. The location of performance for this contract will be the offeror(s)' professional office or other place of business. The provision at FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition; refer to the embedded Work Statement for additional information. FAR 52.212-2, Evaluation-Commercial Items applies to this acquisition, specifics are included under paragraph M of the embedded Work Statement. FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition; addenda are indicated in the following solicitation document. FAR 52.212-5, contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition; please note the additional clauses cited by [X] under that clause.
