- DESCRIPTION OF WORK. The Contractor shall provide all personnel, labor, materials, equipment, and services for providing a Telephone-based Nurse Triage Service. The Contractor shall provide clinical assessment and appropriate level of care support services for Fort Irwin TRICARE Prime eligible beneficiaries. The Tele-Nurse Triage Services to callers shall assist beneficiaries in taking responsibility for their personal healthcare needs and concerns. All calls shall be answered by a qualified technician to refer beneficiaries (if necessary) to Registered Nurses following nationally recognized and physician approved clinical protocols and guidelines for symptom based calls - call center guidelines approved by the Weed Army Community Hospital Medical Staff. The service shall include all information from the calls and entered into a database to show assessments, documented resolution, and/or further action needed from a Primary Care Manager (PCM). The Contractor shall communicate with the beneficiaries' Primary Care Manager (PCM) Team located at Weed Army Community Hospital. Items requiring necessary follow-up from Central Appointments or the patient's PCM will be identified by the Contractor.
