1. General This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA247-12-R-0168 and is issued as a Request for Proposal (RFP). This procurement is set-aside for small businesses. The corresponding NAICS code is 811310 (size standard $7 million). 2. Statement of Work: a.INTRODUCTION: The Contractor will provide all maintenance services, both preventative and corrective, for Ralph H. Johnson's ESA HPLC equipment (Serial# CA-911) through a firm fixed price contract with one base year and four option years. b.SCOPE: The Contractor will propose a comprehensive management program for repair and preventative maintenance of the HPLC equipment described as follows: 8-channel Coularray Detector; 542 AS with cooling; 2-582 pumps; and a coularray thermal organizer. c.PERIOD OF PERFORMANCE: 1.Base year: May 1, 2012 - April 30, 2013 2.Option year one: May 1, 2013 - April 30, 2014 3.Option year two: May 1, 2014 - April 30, 2015 4.Option year three: May 1, 2015 - April 30, 2016 5.Option year four: May 1, 2016 - April 30, 2017 d.PLACE OF PERFORMANCE: Medical University of South Carolina, Strom Thurmond Building, 114 Doughty Street, Room 422, Charleston, South Carolina 29425 e.PERFORMANCE REQUIREMENTS: The Contractor will keep or return the HPLC Equipment in good working order through the completion of scheduled and unscheduled repairs as well as preventative maintenance. A 24-hour assistance line will be available for emergent needs, with a response time of 24 hours. 1.Maintenance service includes lubrication, adjustments, replacement of equipment and the temporary rental of substitute equipment. 2.Corrective maintenance coverage includes electrical breakdown, mechanical breakdown, parts, labor, and travel. 3.Preventative maintenance coverage will occur annually at a time of the Medical Center's discretion in accordance with original equipment manufacturer specifications and will include 1-replacement cell, 1-PM kit, collectors, membranes, UV lamps, filaments, jets, multipliers, valves, and dip probes. f.DELIVERABLES: Contractor performance is measured on the ability to provide both corrective and preventative maintenance for the HPLC equipment described in Section 3 and response time to calls for assistance. 4. Applicable Provisions and Clauses This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-056. The following FAR and VAAR provisions and clauses are incorporate into this solicitation document by reference: 52.202-1-Definitions; 52.203-5-Covenant against Contingent Fees; 52.203-6-Restrictions on Subcontractor Sales to the Government; 52.203-7-Anti-Kickback Procedures; 52.204-7-Central Contractor Registration; 52.212-1, Instructions to Offerors-Commercial; 52.212-4, Contract Terms and Conditions - Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (of which 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.223-18, 52.225-1, 52.225-13, and 52.232-34 are applicable); and VAAR 852.246-70, Guarantee. Additionally, please included a completed copy of provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with your offer. 52.212-2, Evaluation - Commercial Items, is also incorporated into this solicitation. The following factors shall be used to evaluate offers: price and past performance. 5. Offeror Attachments So that past performance can be evaluated, offerors must provide information pertaining to at least two successfully completed contracts that demonstrate previous work experience in providing HPLC equipment maintenance services.
