- Revised Solcitation This 1449 has been super seceded in its entirety. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and written solicitation will be issued. This solicitation is issued as Request for Quotation (RFQ) No. AG-82CS-S-12-0011. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC 2005-56. The North American Industry Classification Code (NAICS) for this acquisition is 561720 with a size standard of $16.5 million. The proposal contract is 100 percent set aside for small business. Contract: Janitorial Service for the Columbine Ranger District, San Juan National Forest, CO. Line Item 0001 Base Period Qty:12 Months,UNIT PRICE:__________, EXTENDED PRICE:_____________, Period of Performance: 1 May 2012 through 30 April 2013. Line Item 0002 OPTION PERIOD Qty:12 Months, UNIT PRICE:__________, EXTENDED PRICE:_____________. Period of Performance: 1 May 2013 through 30 April 2014. Line Item 0003 OPTION PERIOD Qty:12 Months, UNIT PRICE:___________, EXTENDED PRICE:_____________. Period of Performance: 1 May 2014 through 30 April 2015. Line Item 0004 OPTION PERIOD Qty:12 Months, UNIT PRICE:___________, EXTENDED PRICE:_____________. Period of Performance: 1 May 2015 through 30 April 2016. Line Item 0005 OPTION PERIOD Qty:12 Months, UNIT PRICE:___________, EXTENDED PRICE:_____________. Period of Performance: 1 May 2016 through 30 April 2017. A site visit is scheduled for 10:00 AM MST 17 April 2012. Offerors shall include prices and descriptive literature containing sufficient technical documentation to establish a bona fide capability to meet this requirement. Only written responses will be considered. Offerors shall submit two professional references for this type of service with the following information: Name/POC Address of POC Phone Number of POC Date of Performance (must be within the last 24 months) Aggregate Value of contract Total square feet serviced Contractor must also specify the proposed position(s) from the area wage determination The Government will award a contract resulting from this solicitation to the responsible offeror who offer, conforming to the solicitation lowest priced, technically acceptable with a positive past performance evaluation. Only those offerors determined to be in compliance with the requirements as stated in this solicitation will be considered for award. If additional information is required to support price reasonableness, each offeror may be required to provide a hard copy of their price break-down. Non-conformance with this requirement may result in the offerors quote being determined unacceptable.
