Description
The Naval Medical Logistics Command intends to negotiate on a sole source basis with Mizuho Orthopedic Systems, Inc., 30031 Ahern Ave., Union City, CA 94587, (949)395-9595, as the only responsible source that can provide an adjustable orthopedic examination and treatment table that can meet the minimum requirements of the facility. This system is for the Naval Medical Center San Diego (NMCSD). The tabletop shall be able to rotate the patient 180 degrees to accommodate anterior and posterior spine procedures. The tabletop shall be at least 80 inches in length. The table shall include power controls for adjusting position, articulation, manipulation, and traction to the patient ™s lower extremities. The tabletop shall be radiolucent. The area below the tabletop shall be unobstructed for 180 degree patient rotation and for imaging capabilities. The table shall be capable of synchronized patient translation to prevent distraction and compression of the spine and to maintain neutral upper body position with adjustable chest slide capability. The table shall be capable of hinging up at least 35 degrees and hinging down at least 20 degrees from a midpoint on the table corresponding with placement of the hips and waist. The table shall be capable of moving at least ±10 degrees in the Trendelenberg position. The table shall have 5-way adjustable arm boards, a removable foot board, adjustable leg sling, and contoured hip and thigh support. The table shall be capable of supporting patient weight up to at least 500 lbs. The table shall be convertible to a full-length surgical table. Electrical requirements are 120 VAC, 60 Hz, and single phase. The system shall be compliant with the Food and Drug Administration (FDA) requirements to market and deliver medical products for use in the United States of America; even should delivery be requested outside of the United States. The claims made for the product shall comply with the regulations of the FDA with respect to products for marketing and delivery of a medical product for use in the United States of America, even should delivery be requested outside of the United States. Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the system shall be in accordance with the OEM terms and conditions. Based on the above information, the only technically acceptable system is provided by Mizuho Orthopedic Systems, Inc. This acquisition is being conducted under FAR Part 13. Provisions and clauses in effect through Federal Acquisition Circular 2005-57 are incorporated. This acquisition is being conducted under simplified acquisition procedures. There are no set-aside restrictions for this requirement. The intended procurement will be classified under North American Industry Classification System (NAICS) 339112 with a Small Business Size Standard of 500 employees. This notice of intent is not a request for competitive proposals and no solicitation document exists for this requirement. However, parties interested in responding to this notice shall submit technical data, including cost, sufficient to determine capability in providing the same product. All capability statements received by closing of this notification of this synopsis will be considered by the Government. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
