- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is CLA-12-R-0015 for Adiana and Novasure. This solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-24. The NAIC code is 334517. 1. A1000 Adiana (6 per box) 4 BX $_________ = $__________ 2. NS2000 Novasure (3 per box) 4 BX $_________ = $__________ The FOB point is Claremore Indian Hospital, 101 S. Moore Avenue, Claremore, Oklahoma 74017. Evaluation will be based on price and past performance. The Contractor shall provide past performance references with name, contact name, period of performance and telephone number. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer.
