- March 30, 2012 All Interested Parties Subject: Group Intervention Services, Solicitation CSOSA-12-R-0025 Attached is solicitation number CSOSA-12-R-0025 which seeks contractors capable of providing Group Intervention Services for the Court Services and Offender Supervision Agency for the District of Columbia. All services offered shall be in accordance with the attached solicitation terms and conditions. This is a Commercial Item Acquisition for services in accordance with Federal Acquisition Regulation Sub Part 12.6 using the following format: • Standard Form 1449 • Part B, Continuation of Block 27a, Standard Form 1449 • Part C, Contract Clauses • Part D, Statement of Work and Attachments • Part E, Instruction to Offerors, Evaluation-Commercial Items Note: This solicitation includes clauses and provisions through FAC 2005-55. Questions regarding this Request for Proposal (RFP) must be submitted in writing via e-mail or fax to my attention no later than April 12, 2012, by 1:00 PM Eastern Standard Time (EST). You may email questions to donna.flecther@csosa.gov or fax them to (202) 220-5711. Offerors must submit their proposal by April 30, 2012 at 1:00 P.M., EST (local). Proposals received after this date and time will be considered late and will not be considered for award. The Government requests that proposals be valid for a period of up to 120 days after submission. You are hereby requested to submit your technical and cost proposals in accordance with the solicitation. Interested parties shall provide the agency with all information required in Part E of the RFP. Failure to provide all required information could result in your proposal not receiving full consideration. The Agency shall issue any and all amendments to this solicitation using the Federal Business Opportunities (FBO) website at www.fbo.gov. It is the offeror's responsibility to monitor the FBO website for solicitation changes. Each Offeror must complete an Online Representations and Certifications Application (ORCA) at http://orca.bpn.gov prior to the proposal submittal date only if online Representations and Certifications were submitted or updated in ORCA within the last year. Failures to provide, submit, or update Representation and Certifications in an ORCA could result in the Offeror's proposal being identified as non -responsive and not available for contract award selection. See Part E.8 for FAR 52.212-3 Offeror Representations and Certifications-Commercial Items (Nov 2011). The Agency reserves the right to award the contract without discussions. Therefore, all proposals should be prepared as your organization's final proposal revision. The offeror shall submit one (1) original and four (3) copies of the technical proposal. One original and one copy of the Cost/Price proposal.
