- List of Provisions and Clauses Applicable to HSFE50-12-Q-0029 Statement of Work - HSFE50-12-Q-0029 Justification for Other Than Full and Open Competition - HSFE50-12-Q-0029 MWEOC Access Information This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and no written solicitation will be issued. This combined synopsis/solicitation is being issued as Request for Quote (RFQ) No. HSFE50-12-Q-0029 for the replacement of a chiller system. The RFQ clauses and provisions are those in effect thru Federal Acquisition Circular 2005-57, effective 03/15/2012. The NAICS code applicable to this requirement is 238220; small business size standard is $14 million. FAR Provisions 52.212-1, 52.212-3, 52.212-4 and 52.212-5 apply to this acquisition. Offerors are to include a completed copy of FAR provision 52.212-3 "Offeror Representations and Certifications--Commercial Items" with their response or provide this information via the ORCA website, http://orca.bpn.gov. The successful vendor must be registered at www.ccr.gov. This competition is for brand name only. Alternate brand names are not acceptable. A Justification for Other Than Full and Open Competition based on brand name specifications is provided as an attachment. Source selection will be based on lowest priced offer. FOB Destination pricing is requested. Please include all applicable Government discounts in your offer. The period of performance will be 120 calendar days from the date of contract award. The Statement of Work and a list of provisions and clauses applicable to this RFP are provided as an attachment to this combined synopsis/solicitation. The Contractor's pricing proposal shall contain an all inclusive cost to provide the specific brand name chiller system equipment, as well as to remove and dispose of the existing chiller system and install the new system in accordance with the Statement of Work. Services are to be performed at the DHS, Federal Emergency Management Agency, Mount Weather Emergency Operations Center (MWEOC) located at 19844 Blue Ridge Mountain Road, Mt. Weather, VA 20135-2006. Contractor and Sub-Contractor service personnel must comply with MWEOC security requirements. Contractors are required to become familiar with CFR 44, Part 15 and ensure any sub-contractors are also familiar with these rules and regulations before entering the MWEOC facility. WEATHER EMERGENCY OPERATIONS CENTER (MWEOC)" for important information regarding access to the MWEOC facility. SITE VISIT - A pre-proposal site visit to answer questions relating to this work effort and a walk-through of the areas where services are to be performed is tentatively scheduled for Thursday, 04-12-2012 at 10:00 AM EST. A maximum of five people (at least one person must be from the Prime Contractor), will be allowed to represent your company and attend the site visit. All Contractors interested in attending the pre-proposal site visit must notify Phyllis Woodward via email at Phyllis.Woodward@dhs.gov no later than 4:00 PM EST on 04-05-2012. The subject line of your email should contain the RFQ number and your email message should include the following information: Company name; Company address; DUNS number; Point of Contact Information (name/phone/email). Further instructions for attending the pre-proposal site visit, including the submission of additional access documents, will be provided as a response to your email. QUESTIONS - Questions regarding this RFQ or the pre-proposal site visit are to be submitted in MS Word format to Phyllis.Woodward@dhs.gov within three business days following the site visit. Hand deliveries and/or faxes of questions will not be accepted without prior approval. Additionally, the subject line of your email shall, at a minimum, contain the RFQ number. PROPOSAL SUBMISSION INSTRUCTIONS - The Contractor shall submit an electronic proposal via email to Phyllis.Woodward@dhs.gov no later than 12:00 PM EST on Thursday, 04-26- 2012. The subject line of emails pertaining to this RFQ shall clearly reference RFQ # HSFE50-12-Q-0029. Total size of email shall not exceed 8MB due to DHS email restrictions. Hand deliveries and/or faxes of proposals will not be accepted without prior approval. Contractor proposals shall be valid for 30 calendar days after the due date for responses to this RFP. A written notice of award or acceptance of proposal, emailed or otherwise furnished to the successful offeror shall result in a binding contract without further action by either party. Amendments to this RFP will be published in the same manner as the initial combined synopsis/solicitation.
