Description
Amendment 3 Amendment No. Three (3) is hereby issued to make the following changes: 1. Revise Paragraph C.2, B, 6, on Page 12 to the following: The Contractor shall deliver dumpster containers at the beginning of the Period of Performance date. Placement of containers shall be identified with the Project Officer described in Section G.2 before containers are delivered. 2. Revise Paragraph C.2, B, 7, on Page 12 to the following: The Contractor shall power wash and deodorized the interior of the 34 yard roll off Solid Waste container on one of the pick-up days once a month from May through September for each year of the period of performance. 3. Revise Paragraph Heading E.2 Contracting Officer Responsibility on Page 16 as follows: E.3 Contracting Officer Responsibility 4. Correct numbering sequence on Page 11 and 12, under Section C.2, Scope of Work, sub-section A, Container Collection Schedule. 5. Add the following statement to the Scope of Work: The Contractor shall be allowed to pick-up and dispose of solid waste/recycled products prior to 7:00AM for easier access to all the containers. 6. Revise the Invitation Memo to the following: Section A, Standard Form (SF) 33, Solicitation, Offer and Award, requires completion of item nos. 12 through 18. Your attention is invited specifically to 15A, 15B, 17 and 18, these items completed; otherwise the IHS will not consider your proposal. Provide a DUNS no. on block No. 15A. Completed proposals are due by 5:00 P.M., MT on April 13, 2012 at the address printed on block 7 of the RFP. 7.Revise the Check List to the following: 1. Standard Form 33 Request for Proposal (RFP) 1 Copy with Original Signature 1 Copy (Page 1-2) 2. Pricing Schedule 1 Copy (Page 6-10) 3. Section K-Representation, Certification & Other Statement of Offerors 1 Copy (Page 26) 4. Certificate of Independent Price Determination 1 Copy (Page 27-28) 5. Small Business and Taxpayer Identification 1 Copy (Page 28-29) 6. Women-Owned Business Other Than Small Business 1 Copy (Page 30) 7. Data Universal Numbering System Provide Number (Page 30, Type on page 1 of the SF-33, Box 15.A) 8. Certificate Regarding Responsibility Matters 1 Copy (Page 33-35) 9. Small Business Program Representation 1 Copy (Page 36-40) 10. Previous Contracts and Compliance Report 1 Copy (Page 40) 11. Affirmative Action Compliance 1 Copy (Page 40) 12. Business Proposal 2 Copies and 1 Original (Page 46) 13. Technical Proposal 2 Copies and 1 Original (Page 46) 8.Sign and date attached file entitled "AMENDMENTS TO SOLICITATION" and submit with Request for Proposal packet.
