Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and shall not obligate the Government to award a contract. The purpose of this Sources Sought Announcement is to identify potential sources interested and capable of performing the work described herein and obtain information regarding business size and socioeconomic status, for example Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a) HUBZone, Women Owned Small Business (WO), Small Disadvantaged Business (SB), etc.). The North American Classification System (NAICS) Code associated with this announcement is 621610 with a size of standard of $13.5M. The Department of Veterans Affairs Medical Center (VAMC), Richmond, Virginia is seeking potential sources capable of providing Skilled In-Home Hospice Health Care Services. Services will include providing all In-Home Hospice Care Services, as authorized by VAMC, to VAMC authorized beneficiaries in their homes. Skilled services include skilled nursing, infusion therapy, social services, physical therapy, occupational therapy, and hospice care; and shall be provided in accordance with a plan of care developed by the contracted agency, that requires the skill of a licensed professional, specifically authorized by the Richmond VA Medical Center. Skilled In-Home Hospice Services may include the following: Nursing Services, treatment, diagnostic and preventative procedures requiring substantial specialized skills, patient safety monitoring, medication administration and management, including intravenous fluids, medicines, and necessary care of the delivery lines and equipment, performing blood draws for necessary lab work with reports to the physician, patient education and follow-up, teaching, and counseling patient and family members regarding nursing care and other related patient problems, providing a rehabilitation regimen when necessary, evaluation and regular re-evaluation of patient home care needs, making appropriate referrals to other community resources and VA resources, as needed, development, implementation, oversight, and modification of the patient care plan, physical assessment and re-assessment per written policy, documentation of outcomes of care/services provided, ongoing evaluation of any discharge planning needs and appropriateness of hospice care, hospice aide and homemaker services. The following counties and cities are included in the Richmond VA Medical Center servicing area. When issuing information please clarify which of the servicing areas your agency is able to adequately staff. The inability to staff all of the servicing counties and cities should not be a reason not to submit your agency information.
