- Description
- The Great Lakes Acquisition Center (GLAC) is soliciting proposals for Off-site, Outpatient Colonoscopy Services for beneficiaries of the Appleton, Green Bay, and Cleveland Community Based Outpatient Clinics (CBOCs) located in Wisconsin. Contractor shall provide health care providers, supervision, medical facilities, medical equipment, supplies/materials and all administrative functions sufficient to achieve contracted level of care in a manner consistent with the standards of the Joint Commission (JC), or equivalent. Contract Period of Performance is anticipated from May 1, 2012 through April 30, 2013 with one, (1) one-year option renewal period to be exercised at the sole discretion of the Government. Resultant contract type is Firm-Fixed Unit Price, IDIQ contract. The Government may elect to award a single contract or to award multiple contracts for the same or similar services to two or more sources under this solicitation. All qualified offerors/firms who believe they can satisfy this requirement are invited and encouraged to provide a proposal. Offers, including options, will be evaluated for fair and reasonable pricing, technical capabilities to perform services and location, and satisfactory past performance record. Contract award(s) will be made to the responsible offeror(s) who provides the best value to the Government. This procurement is unrestricted and open to all interested parties. The small business size standard for North American Industry Classification System (NAICS) code 622110 is $34.5 Million. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items (if not already completed at https://orca.bpn.gov ). Electronic version of the solicitation will be available on or about March, 28, 2012. Date of receipt of proposals is April 12, 2012 at 11:00 A.M. CST.
