Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation (VA257-12-R-0019) is issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55 (Effective 02/02/2012). This is a total small business set-aside; the associated NAICS code for this procurement is 811219, with and small business size standard of $19 million. This requirement is restricted and only qualified offerors may submit bids. The VA Texas Valley Coastal Bend Health Care System requires the following services: The Contractor shall provide as part of a firm-fixed price contract, all resources necessary to perform the required tasks in accordance with the terms and conditions set forth within solicitation # VA257-12-R-0019, as specified in the attachment. This requirement fulfills the need for routine and emergency maintenance and repairs of VA owned equipment used in the Sterile Processing Department. The Government anticipates award of a base, plus four option year service contract; options are exercised solely at the discretion of the Government. The Contractor shall provide scheduled maintenance and emergency repair service for the items listed in B.2, "PRICE/COST SCHEDULE" of the attached. The Contractor shall ensure proper service levels are maintained and make certain that equipment performance is optimal in accordance with manufacturer specifications. All work must be performed by qualified technician in compliance with applicable manufacturer's recommendations, codes and standards in its entirety. Maintenance and repair (when necessary) shall be performed as indicated in manufacturer's requirements and recommendations for the equipment listed in B.2, "PRICE/COST SCHEDULE" of the attached. The contract will be performed at VA Valley Coastal Bend, Health Care Center at Harlingen (HCC), located at 2601 Veterans Drive, Harlingen, Texas 78550. Offeror must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. In addition to the standard VHA buy terms, the Seller has reviewed the attached solicitation document VA257-12-R-0019, service description and details, and agrees to all terms and conditions set forth in the solicitation document. The awarded seller will be required to complete the attached solicitation document at the time of award. Offers must remain valid for thirty (30) calendar days after close of the solicitation. All questions must be submitted electronically in writing and received by close of business five week days prior to close. No questions will be accepted after this time.
