- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. See attachments through below hotlink under "Additional documentation" to Army Single Face to Industry (ASFI): Combined Synopsis/Solicitation Document, Statement of Work for Line Item 0001 for PHA (medical) and Dental Exams, Appendix A to Statement of Work for Line Item 0001, Statement of Work for Line Item 0002 for Dental Treatment, and Service Contract Act Wage Determination applicable to the locality. The Statements of Work discuss the goals and describe the services generally required by Florida Army National Guard. Appendix A to Statement of Work for Line Item 0001 gives event specific information such as schedule, numbers of Soldiers, specific tasks to be performed. Line Item 0002 Statement of Work for Dental Treatment discusses the intent of the event, authorized treatment codes, number of hours of treatment to be purchased, and contains a disclosure which must be filled out by offerors to be submitted in response to the solicitation.
