Description
1. General This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA247-12-Q-0231 and is issued as a Request for Quote (RFQ). This procurement is set-aside for small businesses. The corresponding NAICS code is 423450 (size standard 100 employees). 2. Product Specifications/Salient Characteristics: The Dialog Plus Hemodialysis machine consists of a flat, color "touch" screen that can swivel nearly 360 degrees for easy viewing. The screen is mounted on a metal housing containing the machine's hydraulic compartment. The electronic components including upper and lower level microprocessors are separated from the hydraulic compartment by dividers, covers, and shelves which protect the electronics from fluid leaks while maintaining their accessibility. The machine can be accessed from the front or rear making servicing the machine and accessing internal components quick and easy. All the blood side components including blood pump, heparin pump, dialyzer holder, and safety devices are mounted on the front of the machine. The IV pole has 4 hooks and the height can be adjusted up or down with one hand. Concentrate jugs on the front shelf. The acid and bicarbonate wands are integrated with concentrate lines and disinfected with the rest of the dialysate fluid path. 4. Applicable Provisions and Clauses This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-056. The following FAR and VAAR provisions and clauses are incorporate into this solicitation document by reference: 52.212-1, Instructions to Offerors-Commercial; 52.212-4, Contract Terms and Conditions - Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (of which 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.223-18, 52.225-1, 52.225-13, and 52.232-34 are applicable); and VAAR 852.246-70, Guarantee. Additionally, please included a completed copy of provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with your offer. 52.212-2, Evaluation - Commercial Items, and VAAR 852.211-73, Brand Name or Equal, are also incorporated into this solicitation. At minimum, the offered item must meet all requirements set forth in the product specifications/salient characteristics section to be considered a responsive offer. The offeror that submits the offer that meets all the requirements of the solicitation, is responsive and responsible, and has the lowest cumulative price for all items (inclusive of delivery) will be selected. 5. Offeror Attachments If an "or equal" item is offered, the offeror must submit descriptive literature that conclusively establishes product equivalency and technical acceptability at time of quote submission. Also, if the offeror is not the manufacturer of the quoted items, the offeror shall submit written evidence at time of quote that clearly verifies that his firm is an authorized distributor and/or reseller of the quoted products and will provide the items on or before the specified delivery date. 6.
