Past Performance Questionnaire Sample Client Notification Letter Sample Bank Notification Letter Business Management Questionnaire Statement of Work RFP-TDAT-058-12 Newport News The Federal Bureau of Prisons, TDAT Contracting Office, is seeking offers from sources having the ability to provide community-based outpatient sex offender treatment services, within a 10mile radius of the United States Post Office, 10830 Warwick Blvd., Newport News, VA 23601, and within ½ mile of local public transportation stops. Performance periods under this contract will be for a one-year base period estimated to begin on July 1, 2012 with four (4) one-year option periods, with services ending (if all options years are renewed) on June 30, 2017. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. The request for proposal will be available on or after March 21, 2012 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the proposal will not be available. The site provides downloading instructions. All future information about this acquisition, including proposal amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about April 20, 2012. Faith-Based and Community -Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. The BOP anticipates awarding a firm-fixed price contract. The solicitation will be set-aside 100% Small Business. All responsible small business concerns may submit an offer which will be considered by this agency.
