- Performance-Based Work Statement WD No: 1996-0224 Rev 27 Price List This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This solicitation is issued as a Request for Quote. This is a 100% Small Business Set-Aside Competition with a North American Industrial Classification Systems (NAICS) of 562112. The associated NAICS of 562112 has a Small Business Size Standard of $12.5m. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number FA8623-12-A-6808 is issued as a Request for Quotations (RFQ) and is conducted as a commercial item procurement using Simplified Acquisition Procedures (FAR Part 13.303, BPA) for Non-Personal Service (Performance-Based Service Acquisition for Hazardous Waste Pick-Up, Transportation and Disposal Service. The ASC/WNVK intends to establish a single-award Blanket Purchase Agreement (BPA) to fulfill this requirement; however, the Government reserves the right to not award any BPA and does not guarantee any order placements. The resultant BPA award will be five (5) years in duration or less. DESCRIPTION OF AGREEMENT Contractor shall furnish Hazardous Waste Pick-Up, Transport, and Disposal service for five (5) years or less to the ASC/WNV Det 1, Air Force Plant 42, Palmdale CA, as stated in Performance Work Statement (PWS) (see attachment 1). Requirements shall be furnished if and when requested by the Contracting Officer or duly appointed authorized Government Caller. Any service call will be paid using the government purchase card or electronic funds transfer. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC), Number 2005-57 and Defense Acquisition Circular (DAC): DPN 20120312. This RFQ has 19 types of NON-RCRA Hazardous Waste (see attached PWS Appendix A). The contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services necessary to perform Pick-up, Transport and Disposal as stated in the PWS. Quotations shall be accompanied by a completed Representations and Certifications (Reference: FAR 52.212-3). The Government's preferred method is electronic Representations and Certifications. This can be accomplished by registering in ORCA at https://orca.bpn.gov and completing the on-line Representations and Certifications. Contract financing is NOT provided for this acquisition. The Government intends to award firm fixed price (FFP) BPA of non-personal service resulting from this solicitation to the responsible offeror(s) whose quote is the lowest price and passing past performance result. The Government intends to award without discussions and the Government reserves the right not to make an award at all. By submitting a quote, the offeror acknowledges the requirement that potential awardees must be registered in the Central Contractor Registration (CCR) prior to award which can be accomplished at https://www.bpn.gov/ccr/default.aspx. Quotations may be in any format or use attachment two (Price List), including: proposing company's name, address and phone; point of contact's name, phone, and email; quotation date; quotation number; total price; any discounts terms; how long the price list is valid for. Quoted price shall be FOB DESTINATION. Delivery location of service is at ASC/WNV Det 1 Air Force Plant 42, Palmdale CA 93550. The following Federal Acquisition Regulation (FAR) provisions apply to this acquisition: 52.212-1, Instructions to Offerors – Commercial Items(June 2008) 52.212-2, Evaluation Factors. The evaluation factors for the fill-in portion of this provision will be handled in accordance with FAR 13.106-1 and 13.106-2. Evaluation criteria will be lowest price and past performance. Past performance criteria will be passing or fail base on Past Performance Information (PPI) collected.
