This is a combined synopsis/solicitation for commercial services and/or supplies prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this requirement is VA259-12-Q-0222 and is hereby issued as a Request for Quote (RFQ) using subpart FAR Part 13, Simplified Acquisition Procedures. The VISN 19 Rocky Mountain Acquisition Center, 4100 E Mississippi Ave. Glendale, Colorado 80246 on behalf of the Cheyenne VA Medical Center, 2360 E. The North American Industry Classification System (NAICS) code is 423450 “ Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers and the Small Business Size Standard is 100. This procurement will be a 100% small business set aside. Women owned, service disabled veteran owned, veteran owned and small disadvantaged businesses are strongly encouraged to submit quotes. The Government intends to issue a firm fixed price contract to the responsible Offeror who proposal provides the best value for the Government based on price, past performance and technically acceptable proposal which conforms to the solicitation. Only technically acceptable offers will be considered. The Government intends to evaluate offers and make award without discussions; however, this does not preclude the Government from having discussions if it is in the best interest of the Government. All interested parties must bid on all items. All salient characteristics of the IPC ® and the hand tools offered must be equal to that of the MEDTRONIC IPC ®. If a brand name other than Medtronic is offered, training may be required for medical staff (for use) and/or biomedical staff (for Maintenance). The selected contractor shall provide all items requested per the requirements. These items shall be delivered to the Cheyenne VA Medical Center, Warehouse, 2360 E. Pershing Blvd, Cheyenne, WY 82001. Expected delivery is 30 days ARO or earlier. Offerors must comply with all instructions contained in provision FAR 52.212-1, Instructions to Offerors-Commercial Items. The following additional provisions and clauses apply to this acquisition: 52.212-3, Offeror Representations and Certifications- Commercial Items, a completed copy of this provision shall be submitted with quote unless the offeror is registered in ORCA, which must be noted in quote; 52.212-4, Contract Terms and Conditions - Commercial Items; 52.212-5, Contract Terms and Conditions required to implement Statutes or Executive Orders - Commercial Items, to include clauses 52.204-10, 52.211-6, 52.219-6, 52.219-28, 52.222-3, 52.222-19, 52.222-36, 52.223-18, 52.225-1, 52.225-13 and 52.232-34. Attention: You must be registered in CCR to be considered for award. The following clauses apply: FAR 52.204-7, Central Contractor Registration (Oct 2003) (B)(2) the offeror shall provide their DUNS or, if applicable, its' DUNS plus four number with its offer, which will be used to verify that the contractor is eligible for award.
