- SOURCE SOUGHT THIS IS A SOURCES SOUGHT NOTICE ONLY. This is not a request for proposal. There will not be a solicitation issued at this time. The Department of Veterans Affairs, Medical Center (VISN) 16 - Oklahoma City, Ok, has a requirement for labor, materials, equipment and supervision to perform project #635-406, Construction for SICU at the Oklahoma City VA Medical Center. The Government seeks to award a contract resulting from a solicitation to a responsible offeror whose proposal will conform to the solicitation and provides the best value to the Government with price and other factors considered. Note: Offerors without previous Project Experience as the Prime Contractor involving construction within and above an occupied hospital and experience involving the addition of a new occupiable floor on top of a building that is seven (7) stories or higher will NOT be considered eligible for award. Note: If you feel you meet the above criteria, please provide proof of your project experience by completing the information below: a.Project Experience - Demonstrated capability to perform projects of similar scope and size. i.Offeror may only present projects for which they are the listed prime contractor ii.The projects shall indicate experience that includes construction work on multi-story structures that are seven (7) stories tall or higher and within occupied medical facilities "State whether the project was at a federal agency or for private sector "Provide contact information for the contracting officer or owner's representative. "State whether construction was performed within/above an occupied hospital. "Provide both the contractual and actual start and finish dates. "State the gross square footage of the building addition portion of the project. "State whether construction project was within an occupied medical facility indicate if it was related to an ICU unit (i.e. SICU, MICU, CICU, or PICU etc.) or other facility space. "State whether construction project involved construction on top of a multiple story building seven (7) stories or greater. a.Please provide the customer, owner or owner's representatives contact information (Business name, contact name, business address, phone number(s), email address) The Government will assess the degree of success of the Prime Contractor's past performance in relevant projects, targeting areas covered in the requirements of this project.
