- ****** THIS REQUEST FOR QUOTE FA3047-12-T-C138 IS CANCELLED IN ITS ENTIRETY AND WILL BE REPLACED WITH A REQUEST FOR PROPOSAL (RFP) SOLICITATION ON OR ABOUT 23 MARCH 2012. ********* This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in Federal Acquistion Regulation (FAR) Subpart 12.6, and in conjunction with FAR Part 15 as suppl emented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ) and the reference number is FA3047-12-T-C138. This solicitation document with the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55 and DFARS change notice 20120130. Price will include delivery/transportation cost. Quote will be valid until 31 May 2012. Written quotes are required (oral offers will not be accepted). Submission of quotes must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Items. Award will be made to the Low Price Technically Acceptable (LPTA) responsible offeror whose quote conforms to the solicitation and whose sample is technically acceptable. Please include CAGE Code, DUNS, Tax Identification Number, and Discount Terms. This is a 100 percent small business set aside. The NAICS Code is 315999 and the size standard is 500 employees. (CST) to: 802 CONS, 1655 Selfridge Avenue, Attn: Ms. Guadalupe Murphy, Lackland AFB TX 78236-5253. The sample will be evaluated by the government to ensure it meets all technical requirements (see attachment for salient characteristics and evaluation criteria) prior to contractor manufacturing all items. Items must be manufactured in compliance with Berry Amendment USC, Title 10, Section 2533a. Samples are pursuant to FAR 52.212-1(d). FOB: Destination. Upon technically acceptance of the Vest, Plate, and Cummerbund sample by the Government, Delivery for the remaining quantity (Vests - 739 each; Cummerbunds - 739 each; Plates - 739 pairs) will be to: BMT Warehouse, Bldg 5189, Lackland AFB TX 78236-5253. The Government required delivery is 60 days after receipt of the award. To be eligible to receive an award resulting from this RFQ, Offerors must be registered in the DOD Central Contractor Registration (CCR) database, no exceptions.
