- Amendment 003 The purpose of Amendment 003 is to change the following parts of section L.3.4.3 Past Performance Requirements of the solicitation to read: 1. "Offerors shall submit a list of all Government and Commercial contracts (prime and major subcontracts) in performance during the past 3 years, which are relevant to the efforts required by this solicitation. Data concerning the prime offeror shall be provided first, followed by each proposed major subcontractor, in alphabetical order. This volume shall be organized into the following sections:" (pg. 68) a.The number of years of past performance provided changes from "past 5 years" to "past 3 years" 2."f) Past Performance Questionnaire. The Offeror shall provide the Past Performance Questionnaire from Section J of the solicitation to the representative of the first three (3) contracts cited. The offeror shall request that the representative complete the questionnaire and submit it directly to the Government. Completed past performance questionnaires shall be emailed to The Department of Transportation, Federal Transit Administration, Office of Procurement, 1200 New Jersey Ave., S.E. Washington, D.C. 20590, Rm E42-314, Attn: Mr. Alexander Miller at Alexander.Miller@dot.gov. Questionnaires must be received by 12:00 Noon EST on May 3, 2012" (pg. 70) a.The number of Past Performance Questionnaires required to submit has been changed from all contracts cited to the first three (3) cited under Past Performance.
