- ** THIS CORRECTS THE PREVIOUS PRESOLICITATION NOTICE** The Department of Veterans Affairs, Network 23 Contracting Office, Minneapolis, Minnesota intends to award a firm fixed price contract in accordance with FAR Parts 12 & 17. The North America Industry Classification System Code [NAICS] is 541910, Marketing Research and Public Opinion Polling and the size standard is $14 million. This is a 100% small business set aside. The purpose of this project is to conduct Patient Experience Surveys for the VISN 23 Primary and Specialty Medicine Service Line. The project will involve the management of all aspects of the Patient Experience Survey including collection of the mailed survey responses at a central repository, survey processing and scanning, maintenance of a survey database, development of capacity for users to select and report data from multiple perspectives (e.g. by facility, provider, team, question, date range etc). The contractor should also be prepared to provide data analysis of trends, outliers, and provide education to VISN 23 staff on how to read/utilize the interactive reports. The Government intends to make a single award with a period of performance of May 01, 2012 through February 28, 2013 with four (4) one year option periods to be exercised at the Government's discretion: Option Year 1: March 01, 2013 - February 28, 2014; Option Year 2: March 01, 2014 - February 28, 2015; Option Year 3: March 01, 2015 - February 29, 2016; Option Year 4: March 01, 2016 - February 28, 2017. The solicitation is scheduled to be issued on or about March 13, 2012 and will be issued via Fed Biz Opps Electronic Posting System at http://www.fedbizopps.gov. Offerors are responsible for checking the web site regularly to obtain amendments. Questions regarding this solicitation must be in writing to be received no later than 10 AM local time, March 21, 2012. Vendors are reminded that representatives from their company SHALL NOT contact any VISN 23 personnel to discuss this RFQ during this solicitation process. All questions and concerns regarding this solicitation shall be directed to the Contracting Officer. All responsible sources may submit a response, which if timely received, will be considered by The Department of Veterans Affairs, NCO23, Minneapolis, Minnesota. This synopsis is for notification purposes only and does not constitute a solicitation for bids or proposals. Proposals must be signed and submitted in writing by March 30, 2012 at 10:00 AM local time; be in accordance with the solicitation instructions outlined in the solicitation; and be timely. No web based proposals or faxes will be accepted.
