- Attachment D - Security Requirements Attachment C - Clause Addenda Attachment B - Pricing SOW This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This Notice is issued under Request for Proposal (RFP) 2151120367, using Part 12 "Acquisition of Commercial Items" and Part 15 "Contracting by Negotiation". The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-56. This solicitation will result in a Firm Fixed Price contract for the preventative maintenance and repair services. The contractor shall provide preventative and corrective maintenance on the Heating Ventilation and Air Conditioning system installed throughout the J. Edgar Hoover headquarters building and with optional preventative and corrective maintenance for seven (7) other locations within the Washington Metropolitan area as described in the attached statement of work (Attachment A). The North American Industry Classification System code is 333415 with a size standard of 750 employees. The Government will issue a contract for one (1) base year and four (4) option years for a total contract period of sixty (60) months to be exercised at the discretion of the Government. Proposals are due no later than 4/16/12, 2:00 p.m., local time, to Ms. Almeta Austin, Contracting Officer, Federal Bureau of Investigation, 935 Pennsylvania Ave., NW, Room 6329, Washington, D.C. with reference to RFP 2151120367. Each offeror must send an e-mail notification to Ms. Austin at Almeta.austin@ic.fbi.gov stating its proposal has been submitted by mail. The awardees shall be registered in the Central Contractor Registration Database at (www.ccr.gov). THE HVAC Contractor shall perform any or all functions specified in this contract in accordance with the statement of work provided. All costs associated with preparing proposals shall be the responsibility of the Contractor. Each offeror shall complete an SF-1449, including the pricing sheet (Attachment B). The following shall be applicable to the subject requirement: FAR provision 52.212-1 Instructions to Offerors-Commercial Items (FEB 2012), FAR provision 52.215-5 Facsimile Proposals (OCT1997), FAR Clause 52.212-4 Contract Terms and Conditions-Commercial Items (OCT 2003), FAR Clause 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items (JULY 2005), Each offeror shall include a completed copy of the FAR provision 52.212-3 Offeror Representations and Certifications-Commercial Items (MAR 2005), FAR provision 52.212-2 Evaluation-Commercial Items (JAN 1999) and the Addenda (Attachment C). Each offeror shall adhere to the Security Requirements (Attachment D).
