- Sole Source letter to Metro Medical Equipment & Supply, INC. This is a posting for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442720610318. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-57, Effective 07 March 2012, DPN 20120312, and AFAC 2012-0104, effective 04 January 2012. This acquisition will be solicited as a full and open competition. The North American Industry Classification System (NAICS) code is 339112. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 5047. Requirement Description: Travis AFB intends to award a purchase order for a Smith and Nephew Endobutton ACUFEX Director Elite ACL Drill Guide System for the David Grand Medical Center at Travis AFB - 2 sets. The requirement is as follows. CLIN0001 - Product # 72201746 - Smith and Nephew Endobutton ACUFEX Director Elite ACL Drill Guide System - 2 Sets Please provide the unit cost for each item in the set along with the total price for the whole set. Award shall be made in the aggregate, all or none. Please provide quotes valid until 14 May 2012.
