Description
This is a combined synopsis/solicitation for commercial services and/or supplies prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will NOT be issued. The solicitation number for this requirement is VA259-12-Q-0264 and is hereby issued as a Request for Quote (RFQ). This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-56. The North American Industry Classification System (NAICS) code is 325120-Industrial Gas Manufacturing and the Business Size Standard is 1000. This procurement is unrestricted by business size and socio-economic category and competition is full and open. Women and women owned, service disabled veteran owned, and small disadvantaged businesses are strongly encouraged to submit quotes. This requirement consists of the following items: CLIN 1000-Dry Ice Deliverables/Supply, period of performance date of award through September 30, 2012. The Government intends to issue a firm fixed price contract to the responsible Offeror whose lowest price and technically acceptable quote conforms to the solicitation. Only technically acceptable offers will be considered. The Government intends to evaluate offers and make award without discussions; however, this does not preclude the Government from having discussions if it is in the best interest of the Government. All interested parties must bid on all items. Description: Supply purchase of Dry Ice for purpose of shipping frozen lab specimens to testing location. Sheridan VA Medical Center (VAMC), Community Outpatient Clinics (CBOCs) need Dry Ice to pack in with lab specimens to preserve them as they are transported from the clinic locations to Sheridan VAMC, the parent facility, for testing. The Vendor shall provide Dry Ice and the associated container to each of the CBOCs associated with the Sheridan VAMC as specified. The Dry Ice shall be delivered as needed, but generally not less than weekly. Weekly shipments will vary by CBOC location, but generally range in excess of 250 lbs. to less than 500 lbs. for each respective facility. Vendor will deliver the Dry Ice to the clinic(s) on a determined rotation and on an as needed basis as requested by the clinic(s). Vendor will provide holding container at each clinic site and deliver supply as requested. Vendor shall provide and maintain an insulated Dry Ice storage box at each CBOC at NO additional charge for storage of the Dry Ice. The Dry Ice needs to be stored in a container that is meant to preserve the Dry Ice. Vendor's container should be in good working order to preserve the Dry Ice as expected by industry standards. Any containers not preserving Dry Ice as expected shall be replaced promptly at NO additional charge. The container will sit on the premises in a location determined most appropriate by the clinic and the Vendor. Delivery times will be during regular clinic business hours and days at each respective facility. The Vendor shall not deliver outside of the designated hours. The delivery schedule and times will be mutually agreeable to both parties. The Vendor is responsible for notifying the appropriate Government official at each site for arrangements for the delivery of Dry Ice. If routine delivery is scheduled on a recognized Government holiday, then the delivery must be rescheduled and delivery either within three (3) days before or after the holiday. All work shall be accomplished in accordance with Federal, State, local regulations and Industry Standards. The proposed unit price shall be an all inclusive price that includes HazMat and Energy charges. This requirement is for delivery FOB (Destination) to the Sheridan VA Medical Center CBOCs designated above. Price will be evaluated by the Government for fair and reasonableness. The Government will evaluate quotations for award purposes by adding the total price for all options to the total price. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced, Evaluation of options shall not obligate the Government to exercise the option(s).The period of performance of any resulting contract will be date of award through September 30, 2012.
