Description
The Department of Veterans Affairs Medical Center, Salisbury North Carolina (Salisbury VAMC) is seeking potential sources to furnish all equipment, facility, professional and technical personnel services to provide Homeless Respite (HR) care for eligible veteran beneficiaries as referred by the Salisbury VAMC. This is a potential multiple award contract in which awardees can be located within a 100 mile radius of the Salisbury VAMC, in order to allow patients to receive HR care in a timely, safe and efficient manner without compromising patient care. The Contractor must possess all licenses, permits, accreditation ™s and certificates as required by law to perform these services. Contract services will be provided on an as needed basis and the contract Base Period will be one year from the date the contract is awarded, subject to the availability of FY12 funding, with the option to extend the contract for four (4) one year Option Periods. It is the VA ™s intention to negotiate and award on a competitive basis, pending availability of funds. The Contractor shall provide Temporary Residential Shelter to include but not limited to, beds, restroom, bath, kitchen facilities, and related furnishings. Adequate security measures, e.g., bars on windows and doors and area fenced, in order to provide optimum safety for residents. Provide food for two balanced meals daily and snacks daily to each shelter resident. Contractor to provide personal hygiene items, first aid supplies, and storage as needed. The contractor shall provide services to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs Medical Center herein after referred to as the (VA). The contractor shall provide to the VA, Community Based Health Care for Homeless Veterans (HCHV), in accordance with all terms and conditions, provisions and requirements listed herein for the sum listed above. This rate is inclusive of all services as may be necessary in the treatment of the patient. Unless specifically excluded in this contract, the per diem rate established will include the services listed is this document and will also include all services normally provided other patients by the facility without extra charge. The per diem rate established for emergency services will be paid for all Veterans admitted to the residential program without an initial History & Physical and Mental Health assessment provided by the VAMC Salisbury. Emergency services will be paid until the Veteran has completed follow-up with Primary Care and Mental Health services to not exceed 60 days. The contractor shall furnish services to beneficiaries from whom such care is specifically authorized by the VAMC Salisbury. It is understood that the type of patients to be cared for under this contract will normally require care and services over and above the level of room and board. The contractor will serve Veterans who meet the following definition of homeless: (1) An individual who lacks a fixed, regular, and adequate nighttime residence, or (2) An individual who has a primary nighttime residence that is: (a) A supervised publicly or privately-operated shelter designed to provide temporary living accommodations, including welfare hotels, congregate shelters, and transitional housing for the mentally ill; (b) An institution that provides a temporary residence for individuals intended to be institutionalized; or (c) A public or private place not designed for, or ordinarily used as, a regular sleeping accommodation for human beings. AND/OR (1): Homeless Veterans that are diagnosed with ongoing Mental Illnesses such as: Depression, Bipolar, Schizophrenia, Post Traumatic Stress Disorder, and/or Substance Dependencies. AND/OR (2): 1. Are unable to safely live independently and/or provide for own self-care due to functional, cognitive, and/or psychosocial impairment; 2. Have no suitable significant others able to provide needed monitoring, supervision, and personal assistance; 3. Have complex medical conditions requiring 24 hour supervision and/or care from a VA interdisciplinary home care team; 4. Are not effectively managed independently as evidenced by one or more factors: (a) Impaired mobility due to disability or functional limitations, (b) Inability to manage shelter setting due to cognitive, physical, or psychosocial impairment, (c ) Recurrent hospitalizations or urgent care episodes. The facility will not be used for detoxification or other hospital level treatment, VAMC Salisbury will service as the treatment center. The contractor will employ sufficient personnel to carry out the policies, responsibilities, and the program for the facility. In residential treatment facilities there must be, as a minimum, an administrative staff member or designee of equivalent professional capability on duty on the premises or residing at the house and available for emergencies 24 hours a day, 7 days a week. In those instances where a supervised residential setting is linked to a geographically distinct rehabilitation and/or socialization day program, sufficient professional health care personnel must be identified to provide the necessary therapeutic activities and to ensure a meaningful integration of these efforts with those provided in the residential setting. The NAICS code for this requirement is 623110 Size Standard is $13.5M. This is a sources sought notice only. No formal solicitation document exists at this time. Responses to this notice should include the following: name and address of company/provider, business size (large or small); identify whether the firm falls into any of the following socioeconomic categories “ 8(a), HUBZone, Service Disabled Veteran-Owned; firm experienced in Homeless Respite services. Any other pertinent information which demonstrates firm ™s ability to meet the above requirement.
