Description
The Department of Veterans Affairs Medical Center, Salisbury North Carolina (Salisbury VAMC) is seeking potential sources to furnish all equipment, facility, professional and technical personnel services to provide comprehensive Pain Management (PM) services for eligible veteran beneficiaries as referred by the Salisbury VAMC. The Contractor s place of business must be within a close proximity of the city limits of Salisbury North Carolina, in order to allow patients to receive pain management services in a timely, safe and efficient manner without compromising patient care. The Contractor must possess all licenses, permits, accreditation s and certificates as required by law to perform these services. Contract services will be provided on an as needed basis and the contract Base Period will be one year from the date the contract is awarded, subject to the availability of FY12 funding, with the option to extend the contract for four (4) one year Option Periods. It is the VA s intention to negotiate and award on a competitive basis, pending availability of funds. The contracted PM Specialists shall be board certified or board eligible and be located within the United States. Patients will be walk in patients Monday thru Friday, between the hours of 8:00 am and 4:30 pm at the contractor s place of business. Comprehensive Pain Management is defined as all services required in the delivery of pain treatments, to include, but not limited to, joint and other injections, diagnostic ultrasounds and x-rays and other clinical treatment management. Normal follow-up care associated with the course of treatment is included in the definition of comprehensive pain treatment. Under the terms of this contract, comprehensive pain treatment shall begin when treatment planning begins. Pharmacy, laboratory, specialized imaging (i.e. CT, MRI, PET/CT and Nuclear Medicine) and social services that could be provided at the VA without significant inconvenience to the patient are not included in this definition. Contractor to provide consultations and pain management treatment services to VA beneficiaries referred for evaluation as appropriate to each patient s situation. Cost basis/reimbursement will be by Current Procedural Terminology (CPT) codes. Estimated annual number of procedures is 3,941/yr. The estimated annual number of patients is 1,700. The NAICS code for this requirement is 621498 Size Standard is $10.0M. This is a sources sought notice only. No formal solicitation document exists at this time. Responses to this notice should include the following: name and address of company/provider, number of PM Specialists employed by firm; business size (large or small); identify whether the firm falls into any of the following socioeconomic categories 8(a), HUBZone, Service Disabled Veteran-Owned; firms experience in providing PM services. Information on PM services for current and/or previously preformed for the VA Medical Center and/or other government entities to include contract numbers estimated annual number of procedures and coverage provided per week. Any other pertinent information which demonstrates firm s ability to meet the above requirement.
