- Attachment 2 - Wooden Gate Drawing Size 88 inches X 48 inches Attachment 1 - Wooden Gate Drawing Size 88 inches X 84 inches Request For Quotation 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 - Streamlines Procedures for Evaluation and Solicitation for Commercial Items. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Request for Quote (RFQ) and the solicitation number is SP3300-12-Q-0035 (See attached RFQ). 2. This notice incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-56 effective 02 March 2012 and Defense Federal Acquisition Regulation Supplement (DFARS) DPN 20120305 edition and Defense Logistics Acquisition Directive (DLAD) Current to Revision 5 and PROCLTR 2012-22. The complete text of any of the clauses and provisions may be accessed in full text at http://farsite.hill.af.mil. 3. This Request for Quotation (RFQ) is being issued to establish a firm-fixed price purchase order to provide wooden bracing and blocking gates (sizes 88 inches X 84 inches and 88 inches X 48 inches) for the Defense Logistics Agency (DLA) Distribution San Joaquin, Tracy, CA. 4. Delivery point is FOB Destination. SHIPPING ADDRESS WILL BE AS FOLLOWS: DODACC ADDRESS SB3200 DLA Distribution San Joaquin Supply Branch, Warehouse 29 South End Tracy, CA 95376-5000 5. This RFQ will be awarded on a small business set-aside basis. The North American Industry Classification System (NAICS) code for this project is 321920 and the size standard is 500 employees. 6. DELIVERY SCHEDULE: The specific delivery schedule will be provided to the successful offeror at the time of the award. Quarterly deliveries in the quantities stated below are required for each size gate: 1. Wooden Gate - size 88 inches X 84 inches: 1st quarter - 1,000 each 2nd quarter - 1,000 each 3rd quarter - 1,000 each 4th quarter - 1,000 each 2. Wooden Gate size 88 inches X 48 inches 1st quarter - 400 each 2nd quarter - 400 each 3rd quarter - 400 each 4th quarter - 400 each (Yearly Quantity: 4,000 each, Size 88 inches X 84 inches) 1,600 each, Size 88 inches X 48 inches) 7. PACKAGING REQUIREMENTS Due to insubstantial nature of the OSB component, the potential for product damage during transportation exists; therefore, the Contractor shall ensure gates are protected during the transportation process. In order to facilitate forklift offloading at destination, the Contractor shall strap gates to oversized pallets. The Contractor shall assume the responsibility for determining the number of gates that can be safely nested and strapped to a pallet to ensure safe delivery to destination. 8. PAYMENT INFORMATION: Payment for supplies delivered will be made by the Acquisition Operations Office via the Government Purchase Card (VISA). NOTE: Since all payments under this contract will be made via the Government Purchase Card, the Contractor MUST accept the VISA Credit Card for payment of all supplies under this contract. 9. CENTRAL CONTRACTOR REGISTRATION - As stated in referenced FAR 52.212-1 Instructions to Offerors, paragraph (k) Central Contractor Registration, a prospective awardee shall be registered in the CCR database prior to award, during performance and through final payment of any contract resulting from this solicitation. Offerors may obtain information on registration and annual confirmation requirements via the Internet at http://www.ccr.gov or by calling 1-800-227-2423 or 269-961-5757. INSTRUCTION FOR PAYMENT BY GOVERNMENT PURCHASE CARD This contract/order will be paid by Government purchase card. Invoices for supply contracts shall be submitted at time goods are shipped. Invoices for service contracts shall be submitted in accordance with instructions provided in the contract/order. At a minimum, each invoice shall include the following information: - Contractor name, address, and CAGE code - Invoice date and invoice number - Contract number/order number - Contract Line Item Numbers (CLINs) - CLIN description, quantity, unit of measure, unit price and extended total prices - Additional information required by the Prompt Payment Act at FAR 52.232-25 Unless indicated elsewhere in this contract/order, invoices shall be faxed or emailed to the contract specialist identified on page 1 of this document in the block titled "Administered by". 11. MARK CONTRACT NUMBER ON ALL CORRESPONDENCE Contractor must mark the contract or purchase order number on all paperwork and shipments. The order number must appear on the exterior of the shipment. Failure to follow these instructions will hold up payment to you and could result in the return of merchandise at your expense. 12. UNIT OF ISSUE: EA = Each 13.
