- STATEMENT OF WORK RFI QUESTIONNAIRE All Interested Parties: The United States Department of Justice, Federal Bureau of Investigation (FBI) is issuing this Sources Sought Notice as a means to request information from sources able to provide supplementary medical services, from board-certified physicians, for its field medical support program. Requirements would entail the provision of the following services: • Emergency response and on-site medical support (domestic and international; emergency and training exercises) to FBI missions. • The provision of expert medical consultation, research and support services, and physician medical consultation to pre-hospital providers. • The provision of medical supply and logistical support to the FBI Emergency Medical Support Program at certain FBI facilities including operation of a controlled substances distribution and administration program. This request for information is issued solely for market research purposes and does not constitute a solicitation. Proposals are not being requested and no contract will be awarded from this notice. The information requested will be used to determine whether or not there are qualified sources available to meet the requirements and what types of qualified service providers are available. No reimbursement will be made for any costs associated with responding to this notice. Please refer to the draft Statement of Work for additional information regarding the requirements. Companies responding to this RFI should complete and submit the attached questionnaire. Companies should not submit generic brochures or marketing materials as a response. Please provide a written response to this notice by Monday, March 26, 2012. Responses may be sent directly to Amanda J.
