Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information provided in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, VA251-12-Q-0144, is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-55. Only quotes received by e-mail from offerors are acceptable. E-mail your quotations to Charles.lakin2@va.gov. No telephone or facsimile quotes will be considered. The North American Industry Classification System (NAICS) code for this procurement is 453998 and the business size standard is $7.0 million. All quotations shall state business size. Note: The proposed purchase order listed here is Set-Aside 100% for small business. Line item one (1) of two (2): Contractor shall provide 125ea. Glass Iceburg Table Top Awards with black glossy marble/slate base. "Size of the glass portion of the Iceberg award is 5.5" wide and 6.5" (7.25" including base) tall. In "Individually etched with information to be provided by the VA, two symbols on each, the VA Seal and the symbol for the National Center for Patient Safety (NCPS) and from 50 to 75 characters of text. Note: Wording will be same on all Icebergs with different medical center names. See attached picture. "Gold, Silver, or Bronze award plate shall be affixed to the base of the award starting on the awards right side. oEach plate shall have printed 2011. oPlates shall be either Gold, Silver, or Bronze in color (quantity of each plate to be provided by the VA). oAll plates shall be 1.5" wide by.75" tall Note: The award style specifications have already been established; alternatives will not be considered. Line item two(2) of two (2): Contractor shall provide one (1) Pre-Production proof to the VA and receive written authorization to proceed prior to commencing production. Note: Proof, if approved as provided may be used as part of production quantity in line item one (1). Line items one (1) and two (2) shall be delivered to: VA National Center for Patient Safety, ATTN: Ms. Eviliner Pettes, 24 Frank Lloyd Wright Dr., Lobby M, Suite 2100 POB 486, Ann Arbor, MI 48106-0486. FOB Destination, Terms are NET 30. Pre-production proof must be delivered to the address above NLT 30 days ARO. All questions regarding this RFQ shall be sent to Charles.lakin2@va.gov no later than 3:30 EST on March 30, 2012. Offerors must comply with all instructions contained in provision FAR 52.212-1, Instructions to Offerors-Commercial Items. The following additional provisions and clauses apply to this acquisition: 52.212-3, Offeror Representations and Certifications- Commercial Items, a completed copy of this provision shall be submitted with quote unless the offeror is registered in ORCA, which must be noted in quote; 52.212-4, Contract Terms and Conditions - Commercial Items; 52.212-5, Contract Terms and Conditions required to implement Statutes or Executive Orders - Commercial Items, to include clauses 52.204-10, 52.219-6, 52.219-28, 52.222-3, 52.222-19, 52.222-36, 52.223-18, 52.225-1, 52.225-13 and 52.232-34. Attention: You must be registered in CCR to be considered for award. The following clauses apply: FAR 52.204-7, Central Contractor Registration (Oct 2003) (B)(2) the offeror shall provide their DUNS or, if applicable, its' DUNS plus four number with its offer, which will be used to verify that the contractor is eligible for award. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.
