- Questions & Answers #2 DD Form 254 OPSEC Attachment (page 7) DD Form 254 FOUO Addendum (page 4-6) Draft DD Form 254 (pages 1-3) Performance Work Statement 5 March 2012 FA8224-12-R-0037 Conformed Copy of Solicitation FA8224-12-R-0037 Amendment 0001 Amendment 0001 is posted for this RFP along with a conformed copy of the solicitation. This amendment 0001 replaces the Performance Work Statement, update all CLINs to reflect correct requirement information, revise verbiage in Section L, Instructions to Offers, revise CLIN information for offerors to bid under Section M, Evaluation of Award and replace draft DD Form 254 with corrected draft version. Questions and Answers #2 are also posted. As a correction from previous information, offerors are advised to review FAR 52.215-1, Sections L & M thoroughly...not FAR 52.212-1 or 52.212-2. Proposal due date remains the same for no later than 28 March 2012 at 3:30 MST.
