- Offeror Representations and Certifications Price Sheet This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This solicitation document incorporates provisions and clauses in effect through the Federal Acquisition Circular (FAC) 2005-49. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. The solicitation number for this procurement is AG-02RC-S-12-0033 and is a Request for Quotation (RFQ). Submit written offers only; oral offers will not be accepted. This acquisition is a 100% small business set-aside. The Northern American Industry Classification System (NAICS) code is 424690 and the size standard of 100 employees. A firm-fixed price contract will be issued. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). Information concerning CCR registration requirements may be viewed via the Internet at http://www.ccr.gov or by calling the CCR Registration Center at 1-888-227-2423. DESCRIPTION OF WORK: A quotation is requested for two line items. See attached price sheet. The Successful Quoter is required to provide proof of a valid and current product registration for their company with the U.S. Environmental Protection Agency. Inspection will be made at the delivery points and will consist of random sampling of the product(s) for conformance with specifications and quantity count. Quoter will specify how materials will be packaged for deliveries. LINE ITEM 1: 30,510 pouches of methylcyclohexenone (MCH, anti-aggregating pheromone) - "MCH bubblecaps with quantities making up the total to be shipped to 12 locations in Idaho, Montana, Utah and Wyoming. Pricing shall include shipping costs. LINE ITEM 2: 60,568 each verbenone anti-aggregating pheromone packets with quantities making up the total to be shipped to 28 different locations in Idaho, Montana, Utah, Wyoming and South Dakota. Pricing shall include shipping costs. PROJECT LOCATIONS AND DELIVERIES: A spreadsheet with addresses, quantities and Points of Contact will be provided to contractor at time of award. Required deliveries to be made approximately: April 1, 2012 for LINE ITEM #1 MCH pouches and June 1, 2012 for LINE ITEM #2 Verbenone. All deliveries shall be made on regular workdays (Monday through Friday) during regular business hours (0800 a.m. to 4:00 p.m. local time), excluding official Federal Government holidays. One or more awards may be offered to the best value offer made to the Government, considering price, availability, and delivery schedule.
