- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is being issued as a Request for Quote (RFQ); solicitation number is H92244-12-T-0115. A firm fixed price contract is contemplated. In this solicitation document, the incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-55 effective 02 February 2012. North American Industrial Classification Code (NAICS) 334510 with a standard business size of 500 employees applies to this procurement. This procurement is 100% set-aside for small business concerns. The DPAS rating for this procurement is DO-C9. All responsible sources may submit a quote which shall be considered by the agency. This is a brand name or equal best value procurement, if quoting equal please submit specifications with your quote. The government is requesting a balance treatment system that is portable and uses the Stability Evaluation Test (SET) to objectively measure a person's functional balance control based on a person's posture sway. The evaluation test is required so that the government can objectively determine the need for further testing. The balance treatment system shall measure reaction time, COG movement velocity, directional control, and the various aspects of excursion. The balance treatment system is required to use visual biofeedback as an interface. The government is also requesting a concussion treatment system that employs the use of dynamic visual acuity, perception time testing and gaze stabilization testing that will measure a patient's visual acuity associated with the medical condition of vestibulo-ocluar reflex and that measures a patient's maximum head velocity while maintaining accurate vision. The system is also required to have a comprehensive report generation tool, evaluation note feature, progress report, and daily treatment reports for each patient. Both systems shall not employ the use of wireless components due to security requirements. The contractor shall provide a quote using the schedule in Section B. The following factors shall be used to evaluate offers, in descending order of importance: Technical ability to meet specifications, price, and delivery. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement.
