Description
Synopsis This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Department of Veteran Affairs Manchester Regional Office is issuing this request for quote (RFQ) VA373-12-Q-0012 to seek proposals from wired telecommunication carriers. This solicitation is a small business set aside under the North American Industry Classification System (NAICS) 517110 in which the small business size standard is 1,500 employees. The following commercial services are being requested in this solicitation as detailed in the Statement of Work (SOW). All quotes should be based on minimum requirements listed in the SOW. Description: The Department of Veterans Affairs, (VA), Veterans Benefits Administration, Manchester Regional Office located at 275 Chestnut Street, Manchester, NH 03101 has a requirement to provide local wired telephone services in support of our nation's veterans. The current telephone inventory of the regional office is: The VA intends to award a six month base contract with a period of performance from April 1, 2012 through September 30, 2012 with four (4) one year options to extend services. The contract will be a firm fixed price contract. This solicitation will close on Thursday, March 15, 2012 at 5pm EST at the Manchester Regional Office located at 275 Chestnut Street, Manchester, NH 03101. Questions must be received in writing not later than (NLT) Friday, March 9, 2012 at 12pm EST. Telephonic inquires will not be accepted. Selection will be made based on the offerors ability to meet minimum requirements and best price. STATEMENT OF WORK (SOW) 1. Contractor shall provide the following services: The contractor shall provide all services, materials, equipment, personnel, facilities, transportation and supervision necessary to provide local wired telephone services to the Department of Veterans Affairs Manchester Regional Office located at 275 Chestnut Street, Manchester, NH 03101. INSTRUCTIONS TO OFFERORS Please submit quotes in writing on letter head with tax ID#, DUNS # and be dated with a point of contact. Please provide a detailed firm-fixed price per month for our current inventory of 211 telephone lines. All proposals must be submitted in writing to: Department of Veterans Affairs Attn: Sherri Locke (373/28) 275 Chestnut Street Manchester, NH 03101 or via email to: Sherri.Locke@va.gov All proposals must be submitted no later than March 15, 2012, 5pm EST. End the Statement of Work _____________________________________________________________________________ The Government intends to evaluate proposals and make awards(s) without discussions with offerors (except for needed clarifications as per FAR 15.306(a). Therefore, your initial proposal should contain your best terms, as it pertains to cost/pricing. Do not assume you will have the opportunity to clarify, discuss, or revise your original proposal. At Governments discretion, offerors may be asked for more information or to clarify their offer, request for such information doesn't constitute discussions. However, the Government reserves the right to conduct discussions if the Contract Officer later deems it necessary. The Government will award a contract resulting from this solicitation to the responsible offeror whose proposal conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
