Description
The 138th Fighter Wing requires a contractor to provide all labor, tools, material, and equipment necessary to accomplish the following: 1. The contractor shall provide all labor, material, tools, equipment and transportation necessary to complete the work identified in the plans and this Statement of Work. 2. All debris is to be disposed of off Base. 3. This project is to be in accordance with the MATOC Specifications. 4. This project is to be in accordance with the Environmental Compliance Guide for Designers and Contractors dated 20 April 2010. A copy of this document will be provided to bidding contractors during the pre-bid meeting. 5. The contractor shall apply all safety procedures required by local, state and federal regulations. 6. All work to be performed 7:00 AM to 5:30 PM Monday thru Thursday in accordance with the general conditions stated in the MATOC Specifications. If alternate work schedule is desired it must be requested in writing and approved by CE and Security 48 hours prior to the desired schedule. 7. The contractor shall coordinate all work with a representative of the CE office. 8. Prior to any work the contractor shall submit for approval by CE a minimum of four copies of technical literature for all products to be used. Provide Material Safety Data Sheets for all appropriate products. 9. Refer to any and all sketches, plans, specifications and this SOW included with this Request for Proposal. 10. The contractor will not be responsible for moving furniture or equipment. The movement and replacement of furniture shall be coordinated prior to beginning the work. 11. Perform a condition assessment report IAW Naval Facilities Engineering Service Center's Users Guide (UG) - 2036 - SHR. 12. Prepare surface and apply a "Fuel Resistive Resinous Flooring, 3-Coat System" IAW Unified Facilities Guide Specifications UFGS-09 67 23.15 (August 2010) with references in agreement with UMRL dated January 2012 (copy included). 13. Field verify existing conditions and apply the sections of the specifications that apply to this job application. 14. Floor coating is to be applied to the following room numbers: 108, 110, 113 and 114. A site visit will be held at our location 28 February 2012 at 2:00 PM CST. Requests for Information are due to this office NLT 5 March 2012. Requests for Proposals are now due NLT 12 March 2012.
