- USDA DR2901-001, ITS Space Standards USDA/OCIO-ITS Requirement (USDA form AD-2066 SFO attachment 2) USDA Space Information (USDA form AD-2066 SFO) GSA form 12000 for Pre-lease fire Protection and Life Safety Evaluation for an Office Building (Part A & B) Representations and Certifications (GSA form 3518A) Simplified Lease Proposal Data (GSA form 1364A-1) Proposal to Lease Space (GSA form 1364A) Lease document (GSA form L201A) Request for Lease Proposal (GSA form R101A) Cover Letter This is a combined synopsis/solicitation for commercial office space. This constitutes the only solicitation; Proposals are being requested. The Request for Lease Proposals Solicitation Number is NRCS-WV-12-0001. The USDA/NRCS West Virginia State Office (Government) will be accepting proposals from interested parties for 945 usable square feet of general purpose office space in Tyler County, West Virginia. The space must include all utilities, janitor service, snow removal, lawn care, maintenance services, tenant alterations, and off-street parking are to be provided as part of the lease consideration. Please read the Request for Lease Proposal and all attached documents carefully prior to submitting your proposals. In addition, the space must meet the requirements of the Architectural Barriers Act Accessibility (ABAA) and must not be located in a flood plain. The lease term will be 5 years, with a 5 year renewal option to include a 120-day termination clause. Award will be made to the proposal which provides the lowest price and meets the technical factors required for the space as shown in the Request for Lease Proposal. The Government will also consider moving costs and alterations as a factor during the evaluation process. The Request for Lease Proposal package is attached to this solicitation. Proposals must include the following documentation: Lease Document (Form L201-A) Proposal to Lease Space (GSA Form 1364A) Simplified Lease Proposal Data (GSA Form 1364A-1) Representations and Certifications (GSA Form 3518A) Fire Protection and Life Safety information and documents Pre-Lease Building Security Plan (if applicable) AutoCAD or scaled floor plans delineating the Premises Seismic certification or exemption memo Historic property information and documentation Registration in the Central Contractor Registration (CCR) System USDA DR3901-001; ITS Space Standards This information can also be found on page 8 of the Request for Lease Proposal.
