Attachment F Attachment E Attachment D Attachment C Attachment B Attachment A- Compliance Matrix Request for Proposal Statement of Work OF308 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is ID03120018 and is issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-55. This requirement is a 100% set aside for Service Disabled Veteran Owned Small businesses only. The associated North American Industrial Classification System (NAICS) code for this procurement is 517919 - All Other Telecommunications and small business size standard is $25.0 million. Offeror must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov. The attached Statement of Work provides a description of the requirements including information regarding contract line item numbers and items and dates and places of delivery and acceptance. The selected offeror must comply with the attached commercial item terms and conditions as well as additional contract requirements as found in the SOW. Pre-Proposal Site Survey: The Government intends to conduct a pre-proposal site survey from 9:00AM- 11:00AM EST on Tuesday, March 6, 2012 for offerors to have an opportunity to see existing infrastructure. Offerors are invited to send no more than two (2) representatives to this site survey. Issues clarified as a result of this site survey will be included in the distribution of answers to SOW questions. Offerors interested in attending shall submit, by 2:00PM EST Thursday, March 1, 2012, the name and contact information of their representative to the GSA Project Manager/Information Technology Manager at email address: regina.einhaus@gsa.gov. Parking and traffic concerns at the main VAMC Washington, DC site located at 50 Irving Street, NW, Washington, DC have prompted the Government to urge interested vendors to park nearby at the Armed Forces Retirement Home (3700 North Capital Street, NW) lot which has an entrance at State Gate Road NE off of North Capital Street. Be prepared to take the VA shuttle from that lot, which will bring visitors to the Main Entrance of the Medical Center. GSA and VA representatives will be in the front lobby waiting for interested contractors once they check in at the Security Desk. Representatives who have not previously contacted GSA will not be admitted to the site survey. Questions: Offerors are encouraged to submit questions as early as possible. All questions regarding this requirement shall be submitted in writing via email not later than 12:00PM (noon) EST on Friday, March 9, 2012 to the GSA Contracting Officer, GSA Contract Specialist and GSA Information Technology Manager at the following email addresses: angela.bennert@gsa.gov, ryan.mathews@gsa.gov, and regina.einhaus@gsa.gov. Answers to written questions will be posted in FedBizOpps as amendments to the RFP. Proposals are due on or before 2:00PM EST on March 21, 2012. All proposals must be submitted via email to angela.bennert@gsa.gov, ryan.mathews@gsa.gov, and regina.einhaus@gsa.gov. Award will be made via GSA's automated procurement system (ITSS) https://web.itss.gsa.gov/ Please ensure proper registration in ITSS prior to the proposal due date and time.
