- This modification to solicitation number VA263-12-R-0019 Part Time Orthopedic Surgeon at the VA Black Hills Health Care System, Fort Meade, SD is being issued to include the following website http://www.cms.gov/apps/physician-fee-schedule/license-agreement.aspx into this solicitation. This website can be used to obtain CPT Code pricing. Also please change the date for the receipt of the Past Performance Questionnaires in the body of the letter attached to the Past Performance Questionnaire to March 16, 2012 in lieu of March 17, 2012.
