- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested for the required items and no written solicitation will be issued. Solicitation number W911S7-12-T-0017 is issued as a request for quotation (RFQ).This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55, effective February 2, 2012. This is a Total Small Business set-aside under NAICS 511210 with a size standard of $25,000,000.00. Ft. Leonard Wood MO requires the following items: 0001: DocAve 5 Maintenance for one Front End/Application (Same Server) and one database server - Maintenance for DocAve includes software as well as technical support. Software maintenance components shall include minor releases and service packs for DocAve 5, major releases and upgrades for DocAve 5, and DocAve customization. It shall also include 24/7 live phone support, and email support Monday through Friday 7 am to 7 pm central time. Also includes web support ticket servicing support Monday through Friday 7 am to 7 pm. Period or maintenance shall be 12 months. Offerors must be an authorized AvePoint software reseller. The following Federal Acquisition Regulation (FAR) and Defense Federal Acquisition Regulation Supplement (DFARS) provisions and clauses are incorporated by reference: FAR 52.204-7 - Central Contractor Registration; FAR 52.209-2 - Prohibition on Contracting with Inverted Domestic Corporations-Representation; FAR 52.212-1 - Instructions to Offerors-Commercial Items; FAR 52.212-4 Contract Terms and Conditions-Commercial Items; FAR 52.225-25 - Prohibition on Engaging in Sanctioned Activities Relating to Iran-Certification; FAR 52.233-4 - Applicable Law for Breach of Contract Claim; DFARS 252.232-7003 - Electronic Submission of Payment Requests and Receiving Reports; DFARS 252.232-7010 - Levies on Contract Payments; DFARS 252.246-7000 - Material Inspection and Receiving Report. OFFERORS SHALL PROVIDE COMPLETED COPIES OF FAR 52.212-3 OR ONLINE REPRESENTATIONS AND CERTIFICATION APPLICATION (ORCA) RECORD WITH THEIR QUOTE. ORCA is located at http://orca.bpn.gov. Offerors are required to provide their Contractor's DUNS Number and Commercial and Government Entity (CAGE) code with their quote. The CAGE code entered must be for that name and address provided by the Contractor. Offerors are required to be registered in the Central Contractor Registration (CCR) and willing to register in Wide Area Work Flow. All vendors must have a current Central Contractor Registration (CCR).
