Description
The address is 9501 John J Pershing Drive; Omaha, NE 68112 The area of the room is about 1125 square feet. The perimeter of the room is about 150 lineal feet. Remove existing VCT (Vinyl Composition Tile) including adhesive. We believe the adhesive contains 5% asbestos which must be properly disposed of. The contractor is responsible for any/all asbestos remediation and materials disposal, per attached requirements. Clean existing concrete floor such that it is dry, smooth and free from foreign matter. Layout tiles so that tile joints are 6" from any joint in the concrete. Install new VCT and wall base using the colors shown in the attached.jpeg for the VCT and base per the manufacturer's recommendations. Seal the new VCT and base per manufacturer's recommendations. Prepare two quotes: One that includes asbestos testing, abatement if asbestos is present, remove and dispose of all related materials, per attached requirements. Furnish and install new VCT floor. For this quote show the cost of asbestos abatement as a separate line item and furnish and install new VCT floor as another line item. The other quote includes asbestos testing and no abatement if asbestos is not present. Remove existing floor and furnish and install new VCT floor. Furnish lab test results to the Corps of Engineers representative Brian Hunter at 402-995-2255 and brian.j.hunter@usace.army.mil, or 1616 Capitol Ave; Omaha, NE 68102. The time of performance is 45 days from notice to proceed. Scope of Services ASBESTOS ABATEMENT December 2011 1. GENERAL The floor in the warehouse office at 9501 John J. Pershing Drive (Missouri River Project Office, U.S. Army Corps of Engineers) is made of concrete covered with 12" x 12" floor tiles. The size of the area is approximately 1,200 square feet. The floor tiles do not contain asbestos, but the black mastic adhering the tiles to the substrate may contain 5% chrysotile asbestos. This document describes requirements for removal of the floor tiles and mastic. 1.1 REFERENCES. The following references are applicable for performance of this asbestos removal work. 29 CFR Part 1910Occupational Safety and Health Standards 29 CFR Part 1926Safety and Health Regulations for Construction, especially 29 CFR 1926.1101 Asbestos 40 CFR Part 61National Emissions Standards for Hazardous Air Pollutants 40 CFR Part 763Asbestos 49 CFR Part 171Hazardous Substances 49 CFR Part 172Hazardous Materials Tables and Hazardous Materials Subparts B & C Communications Regulations 49 CFR Part 173Shippers - General Requirements for Shipments and Packaging Subpart M EM 385-1-1 (2008) U.S. Army Corps of Engineers Safety and Health Requirements Manual 178 NAC 22Nebraska Department of Health and Human Services, Title 178 Environmental Health, Chapter 22, Asbestos Projects Nebraska Department of Environmental Quality, Title 129 Air Quality Regulations 1.2 CLASS OF WORK. Activities involving removal of floor tiles and asbestos-containing mastic shall be considered Class II work in accordance with 29 CFR 1926.1101(b). 1.3 NOTIFICATION REQUIREMENTS. Contractor shall provide notification of the project to the Omaha Air Quality Control office and the Nebraska Department of Health and Human Services on the applicable forms. See http://www.cityofomaha.org/pw/environmental-quality/air-quality-control/asbestos and http://dhhs.ne.gov/publichealth/Pages/crl_rcs_asbestos_regs.aspx for requirements and forms. Notifications shall contain all required information relating to the abatement project and shall be submitted at least 10 working days prior to the start of any asbestos-related work. 1.4 BUSINESS ENTITY LICENSE. The Contractor performing asbestos removal work shall possess an appropriate Nebraska license, as required by state regulations. Transportation of asbestos waste shall be provided meeting Nebraska regulations. 1.5 MEDICAL REQUIREMENTS. Asbestos workers shall be provided with medical examinations in accordance with 29 CFR 1926.1101(m) and State of Nebraska regulations. 1.6 TRAINING REQUIREMENTS. Workers shall be trained and certified in accordance with 29 CFR 1926.1101(k)(9) and state of Nebraska regulations. On-site supervisor shall be trained and certified in accordance with 29 CFR 1926.1101(b) "competent person" requirements and state of Nebraska requirements and shall supervise or perform all asbestos-related activities. Copies of training documentation shall be available on-site during work activities. 1.7 RESPIRATORY PROTECTION PROGRAM. The Contractor shall have a written respiratory protection program in accordance with 29 CFR 1926.1101 and 29 CFR 1910.134. All workers required to wear respirators shall be familiar with the contents of the written program. 1.8 HAZARD COMMUNICATION PROGRAM. The Contractor shall implement a written hazard communication program in accordance with 29 CFR 1926.59. 1.9 QUALIFICATIONS OF AIR MONITORING PERSONNEL. Personnel performing air monitoring shall have a minimum of 2 years working experience in the asbestos abatement industry and shall have successfully completed NIOSH course 582 Sampling and Evaluating Airborne Asbestos Dust. 1.10 LABORATORY QUALIFICATIONS. The laboratory selected to perform phase contrast microscopy (PCM) of airborne samples shall have successfully analyzed airborne asbestos samples for the last four consecutive rounds of the American Industrial Hygiene Association (AIHA) Proficiency Analytical Testing (PAT) Program. 1.11 ASBESTOS REGULATED AREAS. The Contractor shall provide suitable barriers to prevent the entry of unauthorized persons into asbestos regulated areas. All personnel entering regulated areas shall have the required training, medical evaluation, and personal protective equipment. Asbestos regulated areas shall be marked with signs in accordance with 29 CFR 1926.1101(k)(7). 1.12 ENGINEERING CONTROLS. Contractor shall employ removal methods that meet State of Nebraska and 29 CFR 1926.1101 requirements for floor tile and mastic removal. Place removed materials in an approved container, handle, and transport in accordance with applicable requirements. 1.13 PERSONAL PROTECTIVE EQUIPMENT (PPE). Contractor shall provide appropriate PPE and require asbestos workers to wear this equipment. PPE shall include coveralls, head coverings, gloves, foot coverings and respirators. Equipment which cannot be decontaminated shall be disposed of as asbestos-contaminated waste. Respirators shall be approved by the National Institute for Occupational Safety and Health (NIOSH) for protection against airborne asbestos fibers. Minimum respiratory protection required is a half-mask air purifying respirator equipped with high efficiency filters for which the worker has a valid fit test. 1.14 EXPOSURE MONITORING. Asbestos exposure monitoring shall be conducted for a minimum of one employee per task being performed during abatement activities. Breathing zone air samples shall be representative of the 8-hour time-weighted average (TWA) exposure. An initial exposure assessment must be conducted, followed thereafter by daily monitoring. Any termination of air monitoring shall be approved by the contracting officer. All samples shall be analyzed by PCM. Conduct all air monitoring in accordance with applicable State of Nebraska regulations. 1.15 DECONTAMINATION. Contractor shall provide a decontamination area for workers and their equipment as required by 29 CFR 1926.1101(j)(2). All equipment and waste containers shall be decontaminated using HEPA vacuuming and/or wet wiping before removal from the area. 1.16 WARNING LABELS FOR DISPOSAL. Containers of asbestos waste shall be labeled in accordance with 29 CFR 1926.1101(k)(8) (OSHA), state of Nebraska, and 49 CFR Parts 172 and 173 (Department of Transportation). 1.17 CLEANUP. After removal of asbestos-containing materials, Contractor shall conduct a final cleanup of the regulated area. Contractor and contracting officer shall conduct an inspection after final cleanup. After the contracting officer approves the final inspection, Contractor shall remove all barriers and signs. 1.18 DISPOSAL AND ASBESTOS WASTE SHIPMENT RECORD. Waste asbestos material shall be disposed of at a landfill approved by EPA and the state of Nebraska for disposal of asbestos. Transportation and disposal shall comply with 40 CFR Part 61, Subpart M, and state of Nebraska requirements. The Contractor shall complete the Waste Shipment Record as specified in 40 CFR Part 61, Subpart M and/or other state of Nebraska waste shipment records. A copy of the completed Waste Shipment Record shall be provided to the contracting officer within 3 days of delivery to the landfill. 1.19CLOSEOUT REPORT. Provide a brief letter closeout report at the conclusion of work, describing the work that was done, dates, location, copies of any air sampling results, quantities and type of material removed, final inspection results, disposal documentation and name of landfill.
