Description
STATEMENT OF WORK A.GENERAL INFORMATION 1. Title of Project: Smart Home Assistive Technology Service Contract 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. 1.Provide one full-time software engineer/developer resident at the Tampa VA for the duration of the contract. 2.Provide one part-time software engineer (.20 FTEE) with a specialty in debugging, error checking and assembly (quality control). This engineer need not be resident in Tampa. 3.Provide one full-time resident biomedical-rehabilitation engineer with experience in Assistive Technologies. 4.Provide one part-time (up to.40 FTEE) behavioral-cognitive psychologist with specific knowledge of Ubisense (UWB) tracking technologies and spatio-fractal statistical analysis methodologies as they relate to changes in behaviors, movement patterns and cognitive status. 5.Provide for 8 weeks of internal training (in total per year) to PTRP staff 6.Provide for replacement parts, equipment and consumables (ie. batteries, cables). 3. Background: The Tampa VA Smart Home, which grew out of a funded project in 2010 has been fully integrated into the Tampa Assistive Technology (AT) program providing cognitive assistive technologies to Polytrauma TBI and other cognitively impaired veterans. The original Tampa Smart Home was designed and developed in partnership with Tampa staff by Ubisense Inc. as part of a contract in 2010. Specifically, Ubisense was contracted to integrate their ultra-wide band tracking hardware with the Smart Home artificial intelligence software (which Ubisense also developed). The Tampa Smart Home is the only such system in existence in the US and the world. 4. Scope of Work: The contractor will also supply materials and equipment as necessary in order for maintenance and develop the Smart Home. The contractor will also provide 8 weeks/year of in house training unless otherwise directed by the Contracting Officer (CO). Dates of training shall be determined in consultation of PTRP staff. If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. 5. Type of Contract: a. Training: firm-fixed-price 2012 b. Personnel: firm-fixed-price 2012 B. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan. 2. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Contractor shall provide the services: a) a one full-time software engineer/developer that will be resident at the Tampa VA for the duration of the contract; b) one part-time software engineer (.20 FTEE) with a specialty in debugging, error checking and assembly (quality control). This engineer need not be resident in Tampa; c) one full-time resident biomedical-rehabilitation engineer with experience in Assistive Technologies; and d) a part-time (up to.40 FTEE) behavioral-cognitive psychologist to assist in developing the behavioral prompting software. The psychologist will have extensive knowledge of the Ultra wide band (UWB) tracking technologies and expertise in analyzing long changes term-behavioral patterns and cognitive status using spatio-fractal analysis methods. The specific development, changes and addition of features shall be determined in consultation with Polytrauma Transitional Rehabilitation Program (PTRP) and AT staff. Payment will be made after each of the following deliverables is verified to be in good working order to provide the funds necessary to complete the next deliverable. The design of upgrades and additional features that will benefit cognitively impaired veterans will be determined prior to installation by the design team which will include the above mentioned Ubisense personnel and select members of the PTRP staff. Deliverable 1: Hiring of above-mentioned personnel as soon as practicable after the award of the contract. The personnel shall work in conjunction with PTRP and AT clinical staff to identify, plan, develop and modify features in the Smart Home. Deliverable 2: Provide 8 weeks in total per year of in house training according to a schedule negotiated with PTRP and AT staff. Deliverable 3: Provide all necessary replacement parts, upgrades and consumables as deemed necessary to maintain the Smart Home AT. E. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. F. REPORTING REQUIREMENTS 1.The contractor shall provide the PM/TM with one update at the completion of training. 2.The contractor shall provide the PM/TM with updates on any technical support requests until technical support is completed. G. TRAVEL [If applicable] Four trips of the part-time programmer trip will be made to the VA Hospital, 8900 Grand Oak Circle, Tampa FL 33637. Date TBD 2012. All costs have been built into Quotation. H. GOVERNMENT RESPONSIBILITIES [If applicable] Government will provide computers I. CONTRACTOR EXPERIENCE REQUIREMENTS (Standard mandatory language for all task orders) The government will determine on a SR basis what positions are considered key personnel. The contractor shall identify, by name, the key management and technical personnel who will work under this task order at the time the work is being negotiated. If a key person becomes unavailable to complete the SR, the proposed Substitutions of key personnel shall be made only as approved directed by the Contracting Officer and the COTR. The government will not dictate specific experience and education requirements of the employees initially proposed to perform the work stated herein. The contractor shall submit a resume of qualifications for the COTR and all other direct employees proposed for the project. All Contractor employees will be approved by the COTR prior to bringing on duty. If, at any time from date of award to the end of the contract, non-key personnel Contractor personnel are no longer available, HES/HIS reserves the right to review the qualifications of the proposed replacement personnel and to reject individuals who do not meet the qualifications set forth in the TLO. Team personnel proposed by the contractor should possess some of the following knowledge and/or skills: The contractor must notify VHA in advance and we will approve or reject proposed contractor key personnel for the performance of this contract. The contractor shall submit a resume of qualifications to the COTR for key personnel and all other direct employees proposed for the project. All Contractor employees will be approved by the COTR prior to bringing on duty. If, at any time from date of award to the end of the contract, Contractor personnel are no longer available, the VHA will approve the qualifications of proposed replacement personnel and will reject individuals who do not meet qualifications set forth herein. The contractor must inform the VHA COTR, and Project Manager/Task Manager when personnel are removed from the contract for any reason. The Contractor shall remove any employee from the performance of this contract within five (5) workdays of receiving notice from the Contracting Officer that the employee's performance is unsatisfactory. All Contractor employees are subject to immediate removal from performance of this contract when they are involved in a violation of the law, VA security, confidentiality requirements and/or other disciplinary reasons. "The contractor must inform the VHA COTR, and Project Manager/Task Manager when personnel are removed from the contract for any reason. "If a key person becomes unavailable to complete the SR, proposed Substitutions of key personnel shall be made only if approved by the COTR and Project Manager. "The contractor shall submit a resume of qualifications to the COTR for all direct employees proposed for the project. All Contractor employees will be approved by the COTR prior to bringing on duty.
